Patient preferences, assessing age-related vulnerability, and balancing treatment benefits with harms are essential to minimizing #undertreatment and #overtreatment in older adults with cancer. @WilliamDale_MD @MelissaLoh21 @myCARG @SIOGorg @AmerGeriatrics @geronsociety @ASCO
Undertreatment & overtreatment of older adults w/ cancer are often discussed, but poorly defined. More rigorous & standardized definitions are needed fal.cn/37KaF #GeriHeme #JCO @cdumonti @WilliamDale_MD

🔍 Over- and Undertreatment at #SIOG2025
At the @SIOGorg 2025 plenary in Ghent, experts tackled a critical question in geriatric oncology: when is treatment too much, too little - and how do we decide?
Chairs William Dale (@WilliamDale_MD) and Clark DuMontier (@cdumonti) presented clear definitions to replace vague labels, emphasizing that decisions must be grounded in geriatric assessment and ethical principles. Speakers showed how vulnerabilities, modifiable risks, and the treatment itself all shape whether care is appropriate.
A key message came from patient advocate Ann Pennella: real quality care happens when evidence, geriatric data, and patient values align.
The session’s takeaway was simple but powerful:
👉 Over- and undertreatment aren’t about the amount of therapy, but whether it’s right for the individual older adult.
