NEW: The Trump admin is using AI to analyze Medicare prior authorizations, and companies get 25% of the "savings" the system generates.
We found Carlyle Group and Patient Square Capital, private equity firms, backing this process.
It's the latest oligarch attack on healthcare.

Replying to @CMSGov
@CMSGov has proposed a pilot program to introduce AI into traditional Medicare which is an exceedingly bad idea. Join me in the call to action to support H.R. 5940 and stop this pilot program.
Medicare will begin piloting the WISeR Model (Wasteful and Inappropriate Service Reduction) on Jan. 1, 2026, across six states (AZ, NJ, OH, OK, TX, WA) for a six-year period.
The model allows contracted private companies to use AI-driven prior authorization within traditional Medicare, even as federal AI guidance and regulatory standards remain incomplete.
The model uses enhanced technologies, such as AI and machine learning, combined with human clinical review to help reduce fraud, waste, and abuse.
Clinicians in participating states can either submit prior authorization requests or have their claims undergo pre-payment medical review.

John references a study last year by @medicarepayment that shows the fed gov’t has overpaid #MedicareAdvantage plans over $590 in the past 18 years.
That sum is more than enough to give physicians a COLA adjustment in the Medicare fee schedule plus a catch up payment since there has been no such adjustment since the Medicare fee schedule was implemented in 1992.

If you thought “You can keep your doctor” applied to the ACA, you are so 2010.
Now it’s the Medicare Advantage lie. Before you sign up for Medicare Advantage in the next month , call your doctor and anyone you get care from and ask them to confirm they are in your Medicare Advantage plan.
And remember. Original Medicare plans , plus Medigap, plus Part D may cost you a little more every month , but when you call your doctor to ask if they take original Medicare , the chances are really good they will be
Don’t get skunked by zero premiums. Medicare Advantage has no Pre Authorization. Many Medicare Advantage plan specialize in it !

NEW: Must-read interview with @OliviaWKosloff on how consolidation in healthcare hurts providers.
"The more consolidated the industry gets, the more difficult it is for independent practices to negotiate for competitive rates, which contributes even more to consolidation."

Medicaid cuts will affect everyone, whether they have Medicaid or not. Because of rising healthcare costs from uncompensated care, those with health insurance will also see their premiums increase.
