On this day celebrating 🇺🇸 independence, 🧵about another kind of "taxation without representation" & how community leaders are ending Wall St's (& their water carriers) colonization of our communities. And, how #web3 can turbocharge return of health sovereignty.

Basic EMR meaning prescribing meds, ordering and resulting labs. That’s it. Imaging and referrals still a goddamn fax mess. And documenting an oral conversation via abridge. Other than that, it’s all apps.ePrescribing is a plug-in. Labs too. So, it’s literally all apps.

Basic EMR = Compliance as a Service. You are right. Everything else is apps.

CMS notwithstanding, we are fixing this on the commercial payer side. @chasedave
As Medicare cuts loom let’s compare fee for service compensation: Veterinarians=cash or credit. Dentist=insurance+your portion cash or credit. Doctors=insurance+0-50$ copay. Since doctors can’t charge more for overhead, the private practice is being replaced by patient mills.

Today marks a brighter future in America now that an executive order on healthcare price transparency has been signed. This issue was never about politics for me, it was about doing the right thing for every patient, worker, employer, and union across the country. The American healthcare system had been robbing all of us by hiding their prices and charging whatever they wanted. It was un-American and unethical. Now, we can celebrate this huge victory and a big thank you to @PowertoPatnts and all that supported

In @GovBraun’s State of the State, he highlighted Melissa Dietrich’s story. She faced a surprise $1,348 bill for a routine checkup & bloodwork. Melissa fought the bill & only had to pay $305. Real upfront prices will give patients like Melissa financial certainty.

Working so well we open sourced how we do it via new non-profit nautilushealth.org -- how to contract in this new model + data tech (metl.dev). Have come a long ways since TED talk 8 years ago but still a long, long ways to go to break out.

Replying to @AlaskaAir
@AlaskaAir Didn't want to be a difficult patient (in seat 1C 4-12-24 flt 405) when flight attendant scolded me for not hearing announcement that a new FAA rule (couldn't find it googling) that you can't hold a tablet on take-off.

Said he had discretion to judge weight (apparently new rule is the tablet can't be over 2 lbs). The maximum weight of the biggest iPad is 1.51 lbs He said he had discretion to judge weight. I empathize with flight attendants dealing with difficult patients but good to know facts.

Incredible reporting by @ChrisDHamby nytimes.com/2024/04/07/us/he… Wait until the class action action lawyers filing breach of ERISA fiduciary cases. That's when real change will happen...like what happened in retirement benefits (2 unanimous Supreme Court victories for employees)

Pro tip: Anytime you hear a highly compensated hospital exec state "no margin, no mission" that's a tell that margin is their mission. Related: This is how you get >100 million Americans in debt to the healthcare system.
