#ESMO26 Dr. @bensolomon1 presents ctDNA data from CROWN: 1L lorlatinib in ALK+ NSCLC. Few samples post-progression but no on-target ALK resistance noted, only bypass / off-target. Those with early progression had more baseline mutations. TP53 and clearance were prognostic.

#ESMO26 Dr. Byoung Cho @cbcbc1971 shows simplified dermatologic prophylaxis in COCOON with 1L SC amivantamab + lazertinib for EGFR+ NSCLC had reduction in dermatologic AEs similar to full regimen - this can reduce treatment burden. Lazertinib lead-in for 1 cycle even better.

Sunvozertinib finally now available in the US - over one year since its FDA approval on July 2, 2025 for EGFR exon 20 insertion positive NSCLC after prior chemotherapy. Approval based on WU-KONG1B with RR 46%, mDOR 11.1 months.

#ESMO26 | PERSEE – Phase III
In advanced NSCLC with PD-L1 ≥50%, adding chemotherapy to pembrolizumab improved median PFS from 6.9 to 16.1 months (HR 0.56). The numerical OS difference was notable (50.2 vs 29.9 months; HR 0.77), but not statistically significant. However, we should remember that OS was not the primary endpoint!
Grade ≥3 toxicity: 53.8% vs 19.3%.
I think adding chemotherapy provides benefit, but at the cost of increased toxicity. We need to better identify the patients for whom this toxicity is worth it.

Pope Leo XIV has been diagnosed with lung adenocarcinoma.
While we had hoped for a different diagnosis, the fact that he was able to undergo a lobectomy is encouraging. Surgery is generally pursued when lung cancer appears localized and potentially curable.
Final pathology will help determine prognosis and whether additional treatment is needed.
Wishing the Holy Father a smooth and speedy recovery.
