This is a typical case of acute occlusion secondary to mid-basilar artery stenosis. Recanalization was achieved after one pass of stent retriever thrombectomy, with residual stenosis of approximately 40%. Adequate antithrombotic therapy during the procedure is critical.

A patient presented with recurrent lower‑extremity weakness. Angiography demonstrated critical stenosis of the left ACA A1 segment and occlusion of the right internal carotid artery. Stent implantation was performed to resolve the stenosis and ameliorate symptoms.
