What a great day at Yale's Advancing Care in Liver Cancer: Surveillance, Team-Based Care, and Emerging Therapies — A Patient and Community Symposium! It was wonderful to bring together patients, families, members of the community, and experts from across our multidisciplinary liver cancer team for a day of informative presentations and engaging discussions. I had the opportunity to speak about integrating liver-directed therapies into multidisciplinary liver cancer care. Throughout the program, there were many thoughtful questions from attendees, leading to valuable discussions about the different aspects of liver cancer care. A particular highlight was hearing from two patients who shared their personal experiences with liver cancer. The multidisciplinary approach is particularly important in liver cancer, where patients may benefit from a number of different treatment options throughout the course of their care. The symposium was also recorded, and we hope to share the presentations with those who were unable to attend. Many thanks to the organizers, speakers, and especially the patients and families who joined us and contributed to such a successful program. #LiverCancer #InterventionalOncology #MultidisciplinaryCare #CancerCare @YaleMed @YaleRadiology @Yale_IR @YaleIMed @YaleDigestive @YaleLiverCancer @YaleCancer @SmilowCancer @MarioStrazzabos @CatherineMezza1 @sundar__raghav

Hot off the press in Current Oncology Reports! As the Interventional Oncology Section Editor, I am pleased to share our latest review, “Interventional Management of Patients with Hepatocellular Carcinoma and Portal Vein Tumor Thrombosis: Are There Differences Between East and West?” by Rong Yan, Tong Sun, Jian Lu, and Gao-Jun Teng. The management of HCC with portal vein tumor thrombosis (PVTT) remains challenging and continues to evolve in the era of modern systemic therapy. This timely review examines important differences between Eastern and Western approaches, including the roles of TACE, HAIC, Y-90 radioembolization, radiotherapy, portal vein stenting, and iodine-125 seed-based endovascular brachytherapy. An important theme is that systemic therapy remains the backbone of treatment, while interventional therapies may have an important role in carefully selected patients as part of a multidisciplinary treatment strategy. The full article can be found at link.springer.com/article/10…. I am pleased to share a recent scientific exchange in the @JVIRmedia regarding our pilot study of single-session transarterial embolization (TAE) with ethiodized oil staining followed by microwave ablation (MWA) for complex central renal cell carcinoma. Jens Borgbjerg and colleagues published a thoughtful Letter to the Editor, and @Ahmed_Farhan97 and I had the opportunity to respond. I have attached our Reply. The original Letter can be found at jvir.org/article/S1051-0443(…. The central question is not whether microwave ablation should replace cryoablation. Rather, it is whether adjunctive transarterial embolization can help overcome some of the historical limitations of heat-based thermal ablation and safely expand its application to carefully selected patients with complex central renal tumors. Our initial study was a small pilot study designed to establish technical feasibility and generate hypotheses, not to demonstrate superiority of one ablation modality over another. The broader question is the role of embolization as an adjunct to thermal ablation, regardless of the ablation modality. Ultimately, this will require larger prospective studies and direct comparative investigation. Thank you to Dr. Borgbjerg and colleagues for initiating an important scientific discussion, and to Ahmed Farhan for his continued work on this project. Hot off the press! Our new review, “Locoregional therapies for intrahepatic cholangiocarcinoma: a stage-stratified, biology-informed multidisciplinary framework,” is now published in Chinese Clinical Oncology. Jonathan Tefera and I review the evolving role of locoregional therapies in the management of intrahepatic cholangiocarcinoma (iCCA), including future liver remnant augmentation, ablation, TACE, TARE, hepatic arterial infusion chemotherapy, and SBRT. Importantly, we discuss how these therapies can be integrated with surgery and modern systemic therapies across the disease continuum. As treatment options for iCCA continue to expand, multidisciplinary patient selection and appropriate sequencing of locoregional and systemic therapies will become increasingly important. Hot off the press in Current Oncology Reports! As the Interventional Oncology Section Editor, I am pleased to share our latest review, “Role of Interventional Oncology in the Management of Neuroendocrine Tumors: 2026 Update,” by Daniel DePietro and @michaelsoulen. The management of neuroendocrine tumor liver metastases continues to evolve, with liver-directed therapies playing an important role as systemic treatment options expand. This timely review examines recent advances in embolization, radioembolization, and ablation, as well as treatment sequencing, novel technologies, and the emerging role of biopsy and molecular profiling. An important theme is the increasingly integrated role of interventional oncology within multidisciplinary NET care, not simply as an isolated treatment, but as part of a longitudinal treatment strategy. Congratulations to Dan and Mike on this excellent and timely contribution! Please join us for our upcoming Advancing Care in Liver Cancer: Surveillance, Team-Based Care, and Emerging Therapies — A Patient and Community Symposium on Saturday, October 17 at Yale. This program will bring together patients, families, members of the community, and experts from across our multidisciplinary liver cancer team to discuss surveillance and early detection, liver-directed therapies, transplantation, emerging treatments and clinical trials, and survivorship. I will be speaking about integrating liver-directed therapies into multidisciplinary liver cancer care and will also moderate a discussion focused on collaborating with our patients to improve care. The multidisciplinary approach is particularly important in liver cancer, where patients may benefit from a number of different treatment options throughout the course of their care. I look forward to what should be a very informative and interactive program. Can we predict how well a liver will regenerate before liver venous embolization? Pleased to share our latest work from the Yale Interventional Oncology Research Lab, now published in the @JVIRmedia entitled “CT Radiomics to Predict Regenerative Biomarker Expression after Liver Venous Embolization: Proof-of-Concept in a Porcine Model.” We evaluated liver regeneration following venous embolization in both healthy and cirrhotic livers and asked whether baseline CT radiomics could provide insight into the underlying biologic regenerative response. Combined portal and hepatic vein embolization (PVE + HVE) produced greater angiogenic activity than PVE alone. Importantly, baseline CT radiomics showed promise for predicting regenerative biomarker expression, with the strongest performance for Ki-67 (AUC 0.84). This is proof-of-concept work, but I believe the direction is important: moving beyond measuring liver growth after embolization toward predicting regenerative potential before treatment. Congratulations to Jana Nguyenová, who led this work, and to Junlin Yang, Xuchen Zhang, @LShewarega, @jomi9601, @ResearchTomKuhn, Bernhard Gebauer, Lynn Savic, and @JuliusChapiro for their outstanding contributions to this collaborative effort.
