💊 HD vs Hemoperfusion: choosing the right extracorporeal modality for toxin removal
Not every toxin is dialyzable—and adsorption ≠ dialysis.
🔹 Hemodialysis: best for small, water-soluble toxins with low protein binding — e.g. lithium, salicylates, toxic alcohols.
🔹 Hemoperfusion: may help with highly protein-bound/lipophilic toxins — e.g. phenobarbital, phenytoin, carbamazepine, valproate.
👉 The choice depends on toxin properties, not just the severity of poisoning.
Know the toxin. Choose the modality. Save the patient.

🐍 Snakebite-associated TMA: VICC vs true TTP — the diagnostic trap
Snakebite + thrombocytopenia + schistocytes ≠ automatically TTP.
🔹 First exclude active VICC: prolonged PT/aPTT, low fibrinogen, ongoing consumption/bleeding.
🔹 Once coagulation normalizes, persistent MAHA + thrombocytopenia + AKI → think TMA.
🔹 ADAMTS13 <10% → TTP → urgent plasma exchange.
🔹 If ADAMTS13 is not severely deficient → snakebite-associated TMA is more likely → supportive care ± KRT.
The key is recognizing when VICC ends and TMA begins.

🐍 Can we remove snake venom extracorporeally?
A key nephrology–toxicology question.
IHD/SLED/CRRT: ❌ Clinically meaningful venom removal unlikely
PLEX: ⚠️ Theoretically possible, but not routine
Hemoperfusion/Hemoadsorption: 🔬 Promising concept, but insufficient clinical evidence
👉 Bottom line: Extracorporeal therapies are primarily for organ support (AKI, hyperkalaemia, acidosis, fluid overload)—not venom clearance.
Antivenom remains the definitive therapy.

Anti snake venom are wonderful drugs
But remember what they CANNOT DO 🐍🐍 🩼

🐍 #snake bite management
Time is kidney
It is important to differentiate between type of envenomation to guide therapy
Eg myotoxic needs fluids first
Rest all need ASV
Also know which will lead to AKI

Had a wonderful time as guest faculty at the PG Teaching Program at @LtmmcM
Sion hospital medicine dept.🩺
Nothing beats the energy of a room full of curious PG students.
Spent the session discussing the clinical nuances of clinical nephrology to medicine fellows…(some hoping will be future nephrologists🤞)
It was lovely to meet my own old mentors❤️

As nephrologist Iam always interested in looking what it does to kidney tissue?
