Anatomy doesn't read textbooks. 11 faces of the cystic artery, one Calot's triangle. The aberrant RHA from SMA is not rare, it's a trap ⚠️. Achieve Critical View 👁️, then clip. The unusual is the usual.

Interesting! I was just thinking about this today and then saw a retweet by @tberzin on 👞 😀

While the PAMORA-RAP trial showed no difference in pain flares, hospitalizations, or endo/exocrine function, it’s a massive step forward.

The UEG Journal best paper prize has been awarded at #UEGWeek 2026 to Mathias Cook for his paper on Naldemedine for preventing recurrent acute pancreatitis. This research brings hope to many patients where this is no preventative options available. Congratulations 👏🏆

Another year of astounding research success at #ACG2026 by our medical students, residents, and fellows! I'm incredibly proud of their hard work. 🎓 #Gastroenterology #MedEd #GITwitter @AmCollegeGastro @ASGEendoscopy @CreightonSOM @Creighton
Grateful for the invitation from ASGNA to speak at "Changing Seasons, Changing Practice." Cold snare polypectomy is a topic I love, and I enjoyed sharing it with such an engaged group of GI nurses! 🌵 #Gastroenterology #GINurses #ASGNA

The scout film is an underappreciated starting point for successful ERCP.
Before the scope goes in:
– Orient with the gastric bubble
– Use 12th rib to identify T12
- Then position L1–L3 at bottom right of image
– Rule out free air *before* you start and save image

Anastomotic leaks are notoriously difficult to manage and often refractory to conventional endoscopic therapies.
Could combining fully covered stents + endoscopic vacuum therapy improve outcomes?

An interesting case series of 5 patients explores this approach.
⚠️ The caveat with small case series: selection and publication bias. Successful cases are more likely to be reported, so results may not reflect real-world effectiveness.

Biliary Anatomy on call:
CBD ≤6mm. Up to 10mm post-chole is ok.
IHBD should be invisible. If dilated = obstruction.
CBD grows ∼1mm/decade after 60.
Red flags: Dilated CBD + IHBD + abnormal LFTs.
Correlate clinically.

R4U Safety Line during Lap Chole
Above the line =😊 Happy surgeon
Below the line =😡 Call your lawyer
The yellow line is not a suggestion. It’s a property boundary
Do NOT cross it unless you want to meet the CBD and Bile Duct Injury
Dissect in the green zone. Fear the red zone

How to do a complete Sphinecterotomy
Books usually teach to keep 1/3 wire to keep inside to do sphincterotomy
But when you go near to the duodenal wall keeping the 1/3 wire inside can injure the extraduodenal wall of CBD and cause perforation. 1/2

Basically they are similar to the numbers on the clock 🕝 with "6" being the lowest segment and on the right
