[Art and Today] ‘Doctor X’ and the Socially Conscious Medical Drama
| Kim Sun-young, Popular Culture Critic
The 2007 MBC drama White Tower is regarded as a work that changed the history of Korean medical dramas. This is because it significantly expanded the themes of medical dramas, which, ever since MBC's General Hospital (1994)—considered the genre's pioneer - had primarily focused on the sense of duty and humanism among medical professionals. Going beyond stories within hospital walls, White Tower took a scalpel to the harsh realities and injustices within the medical field, heralding the birth of Korea's socially conscious medical drama.
A line from the second episode encapsulates the nature of this work. It is spoken by Oh Kyung Hwan (Byun Hee Bong), a professor of basic medicine at Myungin University Hospital, to Choi Do Young (Lee Sun Kyun), a young doctor trying to correct longstanding malpractice and institutional conventions:
"A small doctor treats diseases, a middle doctor treats people, and a great doctor treats the nation."
Applying this maxim to medical dramas, White Tower, which targets the illness of our society, can be considered a work that aspires to be a "great doctor."
Following White Tower, the tradition of socially conscious medical dramas aspiring to become "great doctors" has continued steadily. Notable examples include MBC's Golden Time (2012), which highlighted the fundamental contradictions in the medical welfare system through its focus on emergency medicine; KBS's Good Doctor (2013), set in a pediatric surgery department and addressing issues such as discrimination against people with disabilities and children, as well as for-profit hospitals; and SBS's Dr. Romantic (2016–2023) series, which actively incorporated contemporary medical issues, including controversies surrounding political interference and corruption in the medical field.
Entering the 2020s, however, the production of medical dramas appeared to slow down as the urgency of real-world medical issues became increasingly pronounced, particularly due to the COVID-19 pandemic and the medical crisis. Amid this trend, Netflix's The Trauma Code: Heroes on Call (2025) made its presence felt by portraying the current state of a medical industry dominated by capitalistic logic and the realities faced by medical professionals as healthcare workers.
Against this backdrop, SBS's Doctor X: Mafia in White (Doctor X), which began airing last month, is a welcome work heralding the return of the full-fledged socially conscious medical drama.
Like White Tower, this drama inherits the socially critical nature of its Japanese original. During its adaptation, however, it expanded its subject matter to include not only pressing issues in the medical field, such as the introduction of CCTV cameras in operating rooms and the expansion of medical school enrollment quotas, but also darker chapters of Korean history. This has generated considerable repercussions.
In particular, episodes 6 and 7, which dealt with the May 18 Democratic Uprising, presented a groundbreaking approach, even when viewed alongside previous works that have addressed the same historical event.
Unlike earlier dramas about the May 18 Democratic Uprising, which were historical period pieces that directly transported viewers to the actual setting of the events, Doctor X approached Gwangju through the universal and contemporary perspective of a medical drama.
By bringing to light the voice of a victim who had spent a lifetime in Gwangju and trapped in memories of the past, burdened by the "sorrow of the survivors" and a sense of guilt, the drama portrayed the pain of the May 18 Democratic Uprising as a wound that still exists "here and now."
In this respect, Doctor X's approach is one that deserves to be remembered for a long time.
The fact that critics who have stirred up what is commonly called a "political bias controversy" were so taken aback that they exclaimed, "Why bring May 18 into a medical drama?" appears to be a consequence of precisely this impact.
Looking back, addressing historical and political events in socially conscious medical dramas is by no means unusual.
For example, Season 1 of Dr. Romantic dealt with the case involving the alleged manipulation of the cause of death of farmer Baek Nam-gi, while JTBC's Life (2018) drew on the historical tragedy known as the "Seosan Pioneering Corps," which occurred during the Park Chung-hee administration.
At the time, such attempts were praised for broadening the horizons of medical dramas. It is therefore almost contradictory that Doctor X is now being criticized for taking a similar approach.
Doctor X is now faithfully following the path of its noble mission, writing a new chapter in the history of the Korean medical drama genre.

Viewers this year have been captivated by the idea of “justice that delivers instant satisfaction” where corrupt authorities, abusive conglomerates, and heinous criminals are swiftly brought to justice. The “ordinary heroes,” who appear completely ordinary but effortlessly accomplish seemingly impossible feats, have provided television audiences with a powerful sense of catharsis and vicarious satisfaction.
This explains why currently airing dramas such as Doctor X, along with Agent Kim Reactivated, Married Woman Killer, and Teach You A Lesson, have received so much love from viewers.
⭐️Kim Ji Won successfully pulled off a dramatic acting transformation through SBS's Doctor X. She is currently driving the drama to a peak viewership rating of 19.5% through her portrayal of Gye Su-jeong, known as the “mad dog of the operating room,” a character who uses her extraordinary surgical skills to tear down the powerful corrupt forces and injustices within the hospital.
Previously, Kim Ji Won had established herself as a “romance queen” through works such as The Heirs, Descendants of the Sun, My Liberation Notes, and Queen of Tears.
Now, she has given herself a striking new look with a bold short haircut and is delivering exhilarating, cathartic satisfaction to viewers through her unapologetic charisma and confident declaration: “I don't fail.”

A lot of people questioned why there are episodes which tackled issues beyond the patients cases. I think it is to address. Other issues which would confront Gye Su jeongs resolve that Patient's life comes first No MATTER what!
First episode introduced her as a genius surgeon who has a clinical eye on things beyond what is seen (hypersensitivity rxn)
2-4th episode - regardless of conflicts with her medical colleagues, patient's life matters first regardless of status
6-7th - regardless of political battles, patients are patients and doctors must be doctors regardless of how uncomfortable the situation is.
8th -regardless of ones personal battle (medical strike) amd fight for rights, patients life still comes first
In the next episode - it looks like, her own medical license and past is at stake, but it looks like it does not diminish her resolve to save that child

Kim Ji Won Dominates Viewers with Her Surgical Sequences… ‘Doctor X’ Continues Its Unchallenged Lead in TV Ratings.
Actress Kim Ji Won is bringing intense tension to viewers through her surgical sequences.
According to SBS on the 4th, its Friday–Saturday drama Doctor X: The White Mafia Era (hereafter Doctor X) has firmly maintained its No. 1 position in viewership ratings for four consecutive weeks, continuing its unstoppable run at the top with no real competition.
Episode 8, which aired recently, recorded a peak viewership rating of 16.7%, with 15.6% nationwide and 15.1% in the Seoul metropolitan area.
The drama swept all the top spots, ranking No. 1 in its time slot for four consecutive weeks, No. 1 among Saturday miniseries, and No. 1 among all miniseries for the week.
Its 2049 viewership ratings also surged to a peak of 6.09% and an average of 5.4%, demonstrating explosive viewership and completely dominating weekend television.
One of the key factors behind the drama's unstoppable success is undoubtedly the remarkable performance of Gye Su Jeong (Kim Ji Won), famously known as the "Crazy Gye of the Operating Room," as she demonstrates her extraordinary abilities in the surgical theater.
Possessing skills that no one can match, Gye Su Jeong captivates viewers by flawlessly performing surgeries that seem almost impossible. Her god-tier surgical techniques, astonishing speed, and exceptional concentration and physical stamina allow her to accomplish what others would consider impossible.
Gye Su Jeong has proven her overwhelming abilities by successfully performing surgeries on two pancreatic cancer patients consecutively in just six and a half hours. She also prevented a potential tragedy when a patient was at risk of missing the golden hour because there was no doctor available at the hospital to perform a craniotomy. Without hesitation, she made a decisive move and relied on her remarkable surgical skills to save the patient.
Her extraordinary performance reached its peak in Episode 8. To address the medical staff shortage at Guseo University Hospital, she performed three consecutive surgeries over an astounding 12 hours, displaying superhuman determination and concentration.
True to her line, "I never fail," Gye Su Jeong never backs down in the face of any crisis. Instead, she relentlessly searches for a way to save her patients. Her fearless actions transform the drama's intense tension into exhilarating catharsis, delivering an adrenaline rush that keeps viewers hooked.
What makes Gye Su Jeong's presence even more compelling is that behind her extraordinary surgical abilities lies an unwavering conviction that the patient's life always comes first.
This was particularly evident ahead of the surgery on Yoo Ga Eul (played by Ki So Yu), a 10-year-old pediatric cancer patient. Despite being urgently called into another surgery and facing severe time constraints, Gye Su Jeong gave a firm assurance:
"I won't let you suffer for even one more minute."
She then put all her strength into ensuring that the operation would not be postponed.
Viewers have been showering Gye Su Jeong with support and praise for her exceptional surgical skills, as well as her thoughtful consideration and genuine compassion for her patients.
Kim Ji Won's delicate and nuanced portrayal of Gye Su Jeong further enhances the character's appeal. Her intense gaze shining above her surgical mask, controlled delivery of dialogue, and cool, unwavering aura even in the most urgent situations amplify Gye Su Jeong's distinctive charisma and heighten viewers' immersion in the surgical scenes.
Through Gye Su Jeong's remarkable ability to successfully perform even surgeries that seem nearly impossible, Doctor X delivers both the intense suspense unique to the medical noir genre and the exhilarating satisfaction that comes with it.
