This is a brilliant and poignant piece highlighting patient perspectives on overtreatment, clinical trials, and quality of life. I hope many patients read this. evidence.nejm.org/doi/full/1…

So @FDAOncology @US_FDA approves peri operative Durvalumab for resectable NSCLC.
1 - We know there is NO Os benifit for this as of now .
2 - We know that we don't know what is the benifit of adjuvant Durvalumab over neoadjuvant Durvalumab alone .
3 - We know that we need better trial designs in perioperative space.
( which we will never have )
4 - We know that pembrolizumab is already approved in this setting and has shown OS benifit .
5 - And lastly we know that neo adjuvant Nivo alone is probably the most cost effective of all the approved strategies in this space as of today .
Updated OS data from Aegean Trial. After censoring for deaths due to covid 19 - HR for OS is 0.85 ( CI - 0.64 to 1.12 ) . Note that FDA has suggested in its own document that even if OS is demonstrated, it will not show contribution of Neoadjuvant component vs Adjuvant component seperatly . FDA has suggested new designs for future peri operative trials .

We may be living through an unprecedented nightmare hellscape for morality, crime, happiness, the economy, music, movies, science, sports and more.
But at least we can take some solace that we’re living in the golden age of data journalism.

NEWS: I sent a letter to Amgen to investigate the pharma co’s decisions on its cancer drug—after FDA review & new studies suggest its dosing level may be excessive, exposing cancer patients to unnecessarily high costs & harsh side effects from the company’s 8-pill daily dosage.
Thanks for amplifying @SuyogCancer
The mandated dose optimization trial is complete. There’s no advantage to 960.
Optimal dose . Cost of the tablet . Size of the tablet . Sotorasib dosing and Incremental Cost ineffectiveness.
Does this ring bell 🔔 for other oral drugs also? Lorlatinib ? , lapatinib , Abemaciclib ?? , Abiraterone . Comment in @NatRevClinOncol by @garthstrohbehn and Mark Ratain. @VPrasadMDMPH

Access to immunotherapy can be improved in India. We have 100% access at our hospital. It is not possible without low dose and/or mg/kg dose regimens. Access and rationalization of resources can be addressed simultaneously.

Hot off the press! In this new editorial with @garthstrohbehn now out in @JAMAOnc , we caution against ever expanding use of drugs in ever earlier lines of therapy. Are we ready to move immunotherapy use from treating cancers to treating risk factors for cancer? jamanetwork.com/journals/jam…

If you can't muster a simple retweet to:
Denounce terrorism

Offer sympathy and support for dead Jews
...Then there's something profoundly wrong with your sense of justice.
...And your previous advocacy efforts for any marginalized group were empty and fake
All Jews are under attack, because all Jews are one family. Hamas killed over 1,000 Jews bec they were Jews- it has nothing to do with politics. Hamas butchered innocent Jewish men, women, children and babies in the most unimaginable horrific fashion

.@OCCA_Cancer: "It is highly likely that CodeBreak 201 will show that the therapeutic index of sotorasib 240mg daily is superior to that of sotorasib 960 mg daily. Given this probable result, we suggest that the FDA should not grant full approval."
