So what do I do on Monday morning?” 🧠 When should I use intestinal ultrasound—and when is CTE/MRE truly needed—in small-bowel Crohn’s disease? 🚨 Guidelines endorse IUS but remain unclear on escalation, especially in L4 disease & suspected strictures. 📊 CACTUS-CD (prospective, real-world): 🌵🌵🌵 in Red Journal @AmJGastro • n = 407 • Isolated SB disease: n = 149 • Multifocal SB disease: n = 107 • High L4 burden (vs ~3% in METRIC) 🔍 Where does CTE/MRE truly add incremental value? 🎯 CACTUS-CD shows escalation is most useful for: • Proximal & mid small-bowel disease • Surgical planning • Planning endoscopic intervention • Extra-intestinal manifestations (EIMs) • Diagnostic dilemmas / suspected malignancy 💡 Take-home: ✅ Ultrasound-first works for most ➕ SICUS improves stricture detection 🎯 CTE/MRE should be targeted, not routine #IBD #Crohns #IUS #SICUS #Imaging #Guidelines #POCUS @BowelUltrasound @iUSCAN @GENIUS_health1 @saia_gi @AmCollegeGastro @AIGHospitals @DrDNReddy Correlation and Assessment of Small Bowel Lesions Using... : Official journal of the American College of Gastroenterology | ACG journals.lww.com/ajg/abstrac…

✂️ Cut, don't stretch? First RCT of stricturotomy vs balloon dilation in Crohn's Disease 📍 BEST-CD: 106 adults with short (<3 cm) symptomatic CD strictures, randomised 1:1 to endoscopic balloon dilation (EBD) or endoscopic stricturotomy (ES) 🎯 Clinical recurrence at 1 year: 30.1% with ES vs 54.7% with EBD (p=0.01) 🔁 Reintervention: 24.5% vs 49.1% · Emergency visits: 18.9% vs 50.9% · Hospitalisation: 15.1% vs 34% 🔪 Surgery 1.9% vs 11.3% (NS) · Adverse events similar (13.2% vs 22.6%) ⚠️ Expert-centre data, 1-year follow-up: needs multicentre validation 👇 The data in one image 🔗 doi.org/10.1016/j.cgh.2026.1… #MedTwitter #GITwitter #MedEd #IBD #CrohnsDisease #Endoscopy

For short Crohn’s strictures, should we dilate or cut? BEST-CD compared balloon dilation with stricturotomy in a randomized trial. How might these findings influence your approach to a short, accessible stricture? #IBD #Endoscopy

In 106 patients with Crohn’s strictures <3 cm, stricturotomy resulted in lower 1-year clinical recurrence, fewer reinterventions and healthcare use than balloon dilation. Encouraging findings from a single-centre, single-operator RCT—multicentre validation is needed. #IBD #endoscopy

🎓 WISE completed at Asan Medical Center, Seoul! 🇰🇷 An incredible week of interventional EUS: HGS, gallbladder drainage, pseudocyst drainage with HOT SPAXUS, EUS-GJ, TTNB, fiducial placement & RFA. 📸 Special highlight: performing EUS-HGS in pig model! 🙏 Grateful to Prof. Dong-Wan Seo, the Asan team & WEO for their generous mentorship—and colleagues for lasting friendships. ❤️ Thank you, Riya, for inspiring me to pursue WISE when I doubted whether an IBD-focused clinician could take this step. You helped me believe. @WorldEndoOrg @AIGHospitals, Banjara Hills
