It hard to explain why 1 trial with gem/cis +/-PD(L)1 (NIAGARA) is +ve for EFS/OS and 2 very similar trials (presented today) are negative in MIBC. Potentially NIAGARA has the highest risk population, not all respond to IO and the maintenance phase the most effect- but the OS of gem/cis pembro HR of 1 is concerning. This reflects what we saw in metastatic disease with contradictory results for the big combination trials. Overall EVP looks better and more consistent #ESMO26 @myESMO @Annals_Oncology @OncoAlert

More evidence for Nectin4 as a biomarker for EV in @EUplatinum In 1L EV+P, high Nectin4 RNA expression was associated with ⬆️ORR and OS but this was not seen with Pembro alone. Data on membranous expression & amplification might be useful. @OncoAlert europeanurology.com/article/…

An important perspective on the evolving concept of oligometastatic ovarian cancer. The ESTRO–EORTC OligoCare classification provides a valuable framework, but ovarian cancer, particularly high-grade serous disease, poses unique challenges due to its pattern of peritoneal dissemination. This article highlights the need for ovarian cancer–specific definitions and a multidisciplinary approach integrating systemic therapy, surgery, and metastasis-directed radiotherapy. An interesting step toward more precise patient selection and future clinical trial design. @TheLancetOncol thelancet.com/journals/lanon… #OvarianCancer #OligometastaticDisease #GynecologicOncology #Radiotherapy #MedicalOncology #PrecisionOncology

“Natural” does not always mean safe, especially during cancer treatment. A newly published JAMA Oncology Patient Page highlights an issue we frequently encounter in daily oncology practice: the growing use of supplements and “wellness” products during cancer care. Patients may turn to herbal supplements, high-dose vitamins, detox products, or so-called “immune boosters” hoping to improve energy, reduce side effects, or support their treatment. Yet some of these products may: • reduce the effectiveness of anticancer treatments • alter drug metabolism through the liver or kidneys • increase bleeding risk • worsen treatment-related adverse effects Importantly, the message is not that all supplements are harmful. The key question is much more clinically relevant: Is this product safe, evidence-based, and appropriate for this specific patient and treatment? In oncology, we routinely ask patients about prescription medications. Perhaps we should be equally consistent in asking about vitamins, herbal products, supplements, and other wellness products. Because what is marketed as “natural” is not necessarily harmless, and what seems supportive may sometimes interfere with effective cancer treatment. A simple question can make a difference: “Are you taking any supplements or wellness products that we haven’t discussed?” jamanetwork.com/journals/jam… @JAMAOnc #Oncology #CancerCare #MedicalOncology #PatientSafety #SupportiveCare #EvidenceBasedMedicine

Updated results from the phase III OlympiA trial provide compelling long-term evidence for adjuvant olaparib in patients with germline BRCA1/2-associated, high-risk HER2-negative early breast cancer. At a median follow-up of 6.1 years, one year of adjuvant olaparib continued to demonstrate clinically meaningful improvements in: • IDFS: HR 0.65 • DDFS: HR 0.65 • OS: HR 0.72 • 6-year OS: 87.5% vs 83.2% Importantly, the benefit remained consistent across key subgroups, including patients with high-risk hormone receptor-positive disease, with no increase in MDS/AML. These mature data further reinforce the role of germline BRCA1/2 testing and PARP inhibition as integral components of precision treatment in high-risk early breast cancer. annalsofoncology.org/article… @Annals_Oncology #BreastCancer #BRCA #Olaparib #PARPInhibitors #PrecisionOncology #MedicalOncology #OlympiA
