Entry points to hospital. Should more specialties have their own dedicated admissions units? Or should most things come in via a (properly resourced) ED and be sent to the correct specialty from there?

Does the incidence of domestic violence go up in Wales the same way it does in England when the footie is on?

I appreciate this comes across a bit harsh/cynical, and over the years we have had some excellent teaching from other specialties for which I am very grateful.
EM folks: what “top tips” would you give other specialists wanting to “educate” EM about their specialty?
Replying to @Parody_RCGP
Get this is EM too:
Please let us come and educate you about our specialty [so you do more of our work for us, as if you aren’t busy enough already].

A new form ‘only’ takes 1 minute per patient.
650 patients = 650 minutes per day
3,954 hours per year
37.5 hrs per week is 1,575 hours per year.
So we need 2.5 additional staff to fill in a “1 minute” form.
BUT this also works the other way round.
Hey #MedTwitter, can you help?
How do you determine what patient c/o electric shock requires assessment for electrical burn?
All the docs I find say ECG, monitoring, CK, urine myoglobin, fluids, etc.
But surely there's a high proportion that don't need this?
#Burns
Totally agree with @docib here.
If you want to increase efficiency in emergency depts then govt, NHS & Trust Board need to be explicit about how much risk they're prepared to tolerate & indemnify.
Replying to @docib
The words ‘just in case’ are destroying the NHS. Go to A&E ‘just in case’, ‘I’ll get a scan just in case’. If we truly want the NHS to work we need to accept that we can never get it right 100% of the time. This is a very difficult message.

Very well written account of how an excess of caution can contribute to absolutely pointless emergency episodes. Thank you. @acutemedicine
Much more advance care planning is required.

Always in these articles you have anonymous "NHS Chiefs" saying they can't run safe services with this level of funding.
But which trust has said so publicly? None. Without anonymity, which still claim to be safe? All of them.

If an emergency department is crowded then patient should be moved to wards who should create extra capacity. Keeping sick elderly people on waiting room chairs for hours on end is inhumane. Holding all the risk in one place is dangerous and irresponsible.

well-known figure relating to genera practice is that 1/5 of appointments relate to nonmedical issues.
Does anyone know of any research/data looking at this issue in emergency care?
#ENGvSAM
Ford, Farrell, or Smith?
I know who I want, but can England just pick one of them.
It does not need three 'playmakers'. There's so much that needs to be fixed, changing the fly-half every 40 minutes is not the answer.

I beg to differ, you seem to be supporting PA/AAs to the point you're attending their national conferences. I suggest you back away.

If you've never seen a serious heat stroke patient, they look awful.
I mean, truly, horrificly awful
Aggitated, combative, that horrible peri arrest sweat that means nothing sticks to them
But you can, if you get this right save their life.
You need to cool them

I read the @hsib_org report into emergency care yesterday and took some edited "highlights" which will apply to many / most if not all NHS EDs right now - purely from an EM perspective: others will comment on ambulance and in-patient factors 🧵 1/n

After the last few days in ED, I've been thinking about 'spans of control';
How many patients can a Nurse in Charge (of an area) be safely responsible for at any one time?
How can we improve or manage this?

#MedTwitter, I'm helping to deliver a learning session on intubation for new ED nurses, to prepare them for the situation. What should we include? Any tips?
Please RT if you know any ED nurses! 💉
