Started friday morning with organ-sparing partial appendectomy using FTRD for complete resection of an appendiceal pre-cancerous polyp.
Grateful to the incredible Methodist team and my mentors.

Stone Case – Part 2
Impacted CBD stone and basket at the ampulla. Repeat EHL failed. Options were rapidly disappearing. Holmium laser lithotripsy finally fragmented the stone, freed the basket, and avoided surgical CBD exploration.
A fully covered metal stent was then placed.
Endoscopy is humbling. Failed outside ERCP for hard stone disease and distal stricture.
Balloon extraction failed, DASE failed, SpyGlass EHL did not make a dent, Spybasket broke and became impacted alongside stone with no wire access. Options?
Metastatic gastric cancer s/p subtotal gastrectomy with Roux-en-Y anatomy, severe biliary obstruction, and an inaccessible papilla. Performed EUS-guided hepaticogastrostomy, but with only partial relief. Post-Proc day 3, completed antegrade stenting through the HGS.
Busy day with a couple of gastric outlet obstruction cases. One ended up getting a duodenal stent, the other an EUS-guided GJ. Pretty amazing how many different endoscopic options we now have for managing the same problem depending on the situation

Challenging case of ampullary mass with biliary obstruction, cholangitis and inaccessible ampulla. EUS-guided antegrade stenting saved him from PTC. Grateful to my amazing GI team and colleagues who make tackling complex cases possible every day.

Not entirely accurate since Prashant Kedia has pretty much already done it all at Methodist Health System… but hey, when has that ever stopped us from celebrating? Always a good excuse to thank my amazing crew and our industry partners for the constant support.
