We just released the largest real-world study of legal psilocybin services ever conducted.
2,363 participants. 253 facilitators.
Oregon and Colorado. Two years of data from screening through integration.
Thread on what we found..
Most adverse events were mild. Separately, people who'd normally be excluded from clinical trials for psychosis or bipolar history took part safely.
But 11 people with no prior history reported new suicidal thoughts post-dosing.
This is two-week data, not long-term outcomes. That's the next study.

This is biblical.
A woman in her eighties. Ten years into Alzheimer's. Hadn't spoken a full sentence in five years.
Takes one, 5 gram dose of psilocybin.
She slept 19 hours and woke up and spoke for hours about her life, recognized family and held real conversations. She regained bladder control after five years, walked on her own. and dressed herself. Gains held for weeks.
Generally, people on antidepressants asked to taper off of their medicine before they participate in psilocybin assisted therapy. In looking at real world data across Colorado and Oregon's psilocybin programs, we have found that even on SSRIs, the therapeutic effect remains while phenomenological experience is diminished.
AI makes us all powerful. So I made a 404 page video game Easter Egg. We must use these powers for good. Have fun. withalthea.com
Founder work is identity work, executed at speed, with no time for reflection. Psychedelics happen to be unusually good at exactly that combination. Gave a talk on this last week at Boulder Startup Week. @with_althea withalthea.com/2026/05/11/ho…
Been a while.
I haven’t posted here since this platform was still called Twitter. Since then, I left the company I co-founded for a decade, sat with both of my parents through psychedelic experiences, lost my father to cancer, and started building something new.
At @Redox, we helped build infrastructure for modern healthcare.
Now I’m building infrastructure for legal psychedelic care @with_althea.
We started in Oregon and Colorado, where psychedelic therapy is regulated by the state. Today, most legal healing centers in those markets use Althea to manage compliance, outcomes, payments, and client journeys. We’ve now documented 4,000+ psychedelic sessions and support the majority of the regulated market.
What’s become clear to me is that the biggest challenge isn’t awareness.
Millions of people are curious about psychedelics for healing, growth, grief, addiction, depression, creativity, spirituality, or simply feeling more alive again.
But there’s no trusted consumer front door.
People don’t know:
- when it makes sense
- how to do it safely
- who to trust
- what model is right for them
- or how this all fits into healthcare and society
That’s the problem I’m obsessed with now.
I’ll probably start posting more about:
- psychedelic care infrastructure
- healthcare + psychedelics
- consumer trust and marketplaces
- regulation and policy
- what I’m learning building Althea
- and occasionally the deeper personal/philosophical side of all this
If that sounds interesting, like this post so I know you’re still here for the journey.
And if you followed me purely for interoperability APIs circa 2018… no hard feelings ☮️
Live coding demos don't work in the Egyptian room, obviously. #redoxconnect @Redox – at Philadelphia, PA
They wouldn't let me film the goat sacrifice but it was pretty good. #redoxconnect – at Philadelphia, PA
