Breast - Micropapillary carcinoma can show a distinctive HER2 IHC pattern
- Basolateral or lateral, U-shaped membrane staining
- Score as equivocal (2+) when staining is moderate to intense but incomplete (in >10% of tumor cells) → reflex to FISH
- This pattern can be seen in some micropapillary cancers that are HER2 gene amplified (CAP Breast Biomarker Protocol, 2026).

Duster Hybrid - Crisp Analysis
✅ Returned 19 kmpl after 80 km of use in Chennai traffic and highway usage
✅ Engine note is excellent, is eager to perform, feels outright and powerful
✅ Hybrid system works dynamically & very fast, it remains eager to charge the battery and on low speeds that process feels aggressive with a buzz on your pedal & steering
✅ Upshifting feels super smooth but downshifting is slightly noticeable
✅ The way tech works in real time, you feel like this is dirt cheap under 20L with 4-cyl, 1800 cc, 160 ps, 208 nm torque and fuel efficiency on offer.
✅ E-Save mode is good to have to manually charge your 1.4 kWh battery by keeping engine involved most of the time
✅ Precise steering, good feedback, excellent suspension with FDD and overall control. One of the best in segment to drive and with hybrid, no compromise on power in any way. Turning radius felt less too and it was easy to use in traffic.
✅ On low speeds, EV mode helps in good acceleration but regen engage early that you like. Honda City did well there in regen & overall smoothness that we drove a few months back. Duster returned 19 kmpl but it can easily return 20+ in mild traffic with AC because I took a lot of videos/shots with a static car and AC on.
✅ All variants are extremely VFM, I don’t think brand is looking to make any profits initially.
✅ Bootspace isn’t compromised and 7 yr of extended warranty is good to have without paying extra. You just need to stick with service centre for your oil service to fulfil this condition.
✅ Physical buttons, One touch up down windows with anti pinch are highly appreciated.
⚠️ Cabin feels slightly tighter as compared to other cars in this segment like Sierra, Seltos & Creta. Roof is slightly low. Glass area feels average.
⚠️ Downshifting was slightly noticeable and that regen ‘buzz’ on the pedal might irritate some drivers.
⚠️ Renault’s thin dealership & service network and long term ownership cost for this tech after 7 yr is unknown. Warranty on battery is 8 yr
Verdict - 9 out of 10 as a package. Perfect for four as a family car with running above 1000 km looking for fuel efficiency, looks, performance, dynamics and comfort.

📌Post 14 : ADH vs DCIS ; same cells , different diagnosis , and what changed in 2026 🔬
⏩The difference - ADH and low grade DCIS share the same cytologic atypia. What separates them is architectural complexity and extent. ADH involves no more than one terminal duct lobular unit, or a low grade proliferation of about 2mm or less, and larger lesions are called low grade DCIS.
⚫ADH : a high risk lesion, not cancer
⚫DCIS : malignant ductal cells confined to the ducts. It is conventionally treated with surgery, with or without radiation or endocrine therapy.It can progress to invasive cancer, but doesn't always .
⏩The catch on core biopsy:
With limited tissue, the size criteria can't always be applied. Reported upgrade rates for ADH at excision range from 7% to 45.8% . In 236 patients with ADH bordering on DCIS, 46% had DCIS or invasive carcinoma at excision. ( Grabenstetter et. al. )
🆕2026 updates on ADH
➖ASBrS/SBI/CAP guidelines (JAMA Surgery , Aug 2026 ) : most ADH on core biopsy should be excised , though some meeting strict criteria with multidisciplinary consensus can be observed . A second pathology review is recommended, ADH doesn't need negative margins , and patients should get risk assessment and counseling.
➖Miami study ( Arch Breast Cancer , Jan 2026 ) : upgrade was 18% when the core removed half the imaging target or less, vs 2% when it removed more. 48% had low risk features , and all upgrades were to DCIS .
⏩ Keynote:
- ADH marks risk, DCIS is cancer
- Correlate imaging with pathology
- Most ADH gets excised , and select low risk cases can be observed.
- Everyone needs a risk assessment.

GANGLION CELL OR SCHWANN CELL? ONE MISTAKE CAN CHANGE A CHILD’S DIAGNOSIS!
In #HirschsprungDisease, not every stained cell is a ganglion cell!
Ganglion cells: Larger neuronal bodies, vesicular nuclei, prominent nucleoli and abundant cytoplasm.
Schwannian cells: Small, elongated nuclei, scant cytoplasm, associated with nerve bundles.
The immunostain trap:
• Calretinin: Highlights many ganglion cells and intrinsic mucosal nerve fibers.
• SOX10/S100: Highlight Schwannian/glial elements—not proof of ganglion cells.
• HuC/D: Helps identify neuronal cell bodies.
The golden rule: A positive stain is NOT a diagnosis. Morphology + location + immunophenotype matter!
