Aficamten is now approved by the @US_FDA First and foremost - a big thank you to all the patients who invested so much to progress aficamten to this stage. We enrolled the first patient in REDWOOD-HCM in January 2020. Also a big thank you to sites and their research teams who worked year after year on multiple RCTs. Here are the label highlights: -For symptomatic obstructive HCM and with a REMS program - Dose range: 5-20 mg. Initiate at 5 mg. You need a follow up echo anytime between 2 and 8 weeks to check on LVEF/LVOT and do the next titration if needed. No mandates for fixed dose repeated assessments. Maintenance is one echo every 6 months. - Down titration (not stopping) for LVEF 40-49%. Hold if LVEF<40%. - There are few DDI recommendations listed due to CYP2C9 and CYP3A4. The REMS program here allows for prescribing aficamten with the flexibility of rapid or slow titration based on patients symptoms and echo findings, as well as minimize interruptions by incorporating downtitration approach for LVEF in the 40%s. From a scheduling perspective, clinics no longer need to set the echo schedule prior to dispensing the drug given the flexibility in follow up which is a major benefit for clinics like ours (@HongyaChen knows this well!).
