Stats. § 11.1101(1)(a). The name derivatives are only two: “Barry Mandel” (no middle initial) and “Barry R. Mandel.” The first two records share the same residential unit (825 N Prospect Ave, unit 3502) but were entered under different entity IDs, with the punctuation of the address differing (“Ave.. #3502” versus “Ave” plus “Unit 3502”). The third uses the office address of Mandel Group, Suite 600 at 330 E. Kilbourn Ave., where Barry Mandel is chairman and CEO. All three list ZIP 53202; only the third carries a ZIP+4.
