GP surgeries funding has been given to hospitals for them to spaff it up the wall on gaslighting.
Therefore GPs have to make choices on how to allocate resources.
And they do it like no other NHS organisation can.

Four weeks to see a GP is pretty good in my books.
Name me another professional that would do this for 35pence a day and be quicker?
When any left shift funding has gone to trusts for this year already!

Another relentless day sat in front of a screen doing total triage and losing 25 face to face appointments for my patients who will have to wait longer to see me. I didn’t become a GP to look at a screen all day. GPs forced into this by govt. Thanks @DHSCmedia @NHSEngland

Sir Jim Mackey, speaking at the board meeting today, cited improved GP access as a reason for an increase in referrals for secondary care earlier this year. Survey evidence suggests people are finding it easier to make appointments.

They to ask the patient (I'm copied to the letter) to book an appointment to see the GP, to ask for a referral back to Gynaecology.

We overuse the blue nitrile gloves enormously. (I reckon at least 90% of the gloves we use are unnecessary). Standard contact with patients does NOT require gloves and probably increases infection transmission.

Please could people stop asking patients request an expedite letter from their GP. YOU'VE made the clinical decision about when they will be seen, and the delay. Own it and sort it out.

A&G was meant to support GPs, not create another barrier to specialist care.
If referrals are blocked, concerns dismissed and diagnoses delayed, the system risks undermining clinical judgment and patient safety.
Patient safety has to come first.

Hospital to Community
Advice & Guidance hasn’t reduced referrals it has delayed them
The problem is the long waits to see hospital Consultants
The additional work & time is now delaying & reducing GP availability too

NHSE says A&G is not supposed to be a way of rejecting referrals. GPs increasingly think that is exactly what it has become, and I think they are probably right. myemail.constantcontact.com/…

Virtual Wards cost a lot more money than District Nurse and GP input
But let’s move hospitals to communities
This isn’t community care

In case you missed it:
A study of more than 236,000 patients found no consistent evidence that NHS virtual wards reduce hospital admissions, length of stay or readmission rates.

Lots of hospital consultants follow my account. You should take a look at this the next time you try and use block and defect to punch down on GPs.

The report referenced is here in full and makes damning reading vindicating what GPs have been saying about patients coming to harm due to the broken primary-secondary care interface. Three shocking examples hssib.org.uk/patient-safety-…

Referrals new work on single point of access (SpoA model) where specialist advice requests and referrals into a singular digital route. Its been a disaster for patients, a nightmare of unfunded work for GPs, a medicolegal quagmire 🧵
Most GPs have seen a referral rejected inappropriately and nearly one in 10 have seen a patient harmed as a result since the NHS began moving to a 'single point of access' model earlier this year, GPonline polling suggests.
