👉 7 tips to help prevent a ‘long lie’:
Getting stuck on the floor is unfortunately very common for older adults.
Sometimes it’s because the person is injured, but often it’s because they just can’t get themselves up

Now it’s personal
Tell me how is this going to build strength?
My dad can do 25 sit to stands with weighted backpack.
@locala Please let me come and do some training with you.
We need to better exercise prescription, for patients but also to improve efficiency and economics of these services.

THIS!
Loss of muscle mass, power, balance isn’t often just due to aging… it’s usually due to a series of successive periods of less activity due to illness, falls, etc.
Many never get back to baseline after recovery. So the next injury or illness resets the baseline even lower.

The ability to cross-&-step to recover from a sideways loss of balance is one of the most important mobility skills an older adult needs, yet:
⚠️ Its absent from some popular programs
🚩Some programs include practicing it without even confirming pre-requisites
Which is worse?

Oooh, what's this? Just out - more evidence that Bobath is inferior to functional task practice for upper limb recovery to accompany our previous review which found the same thing for the lower limb/ mobility . authors.elsevier.com/sd/arti…
Is anyone still using Bobath? Why?

Frequency of exercise programming cannot be an isolated consideration:
It needs to be considered in context of FITT
If higher intensity exercise (eg 10-15 RM) then non-consecutive days
If low-load skill adaptation, then little & often
Knowledge of RM, RIR & FITT is a must

I personally prefer RIR for assessing exercise intensity rather than RPE… some clients I work with are the other way around
Neither are an exact science and are an approximation… which means a correlation chart can still be useful

Pictures of older adults doing bicep curls with mini weights is so cliche. Like, of all the exercises on my go-to list, isolation of biceps with bicep curls is not even top 10. loaded carries, squats, push-up, deadlift, bent over dumbbell row, step up>>>>>bicep curls.

Exercise can reduce fear of falling in community-dwelling older adults. However, many exercise principles (e.g., progression, reversibility, overloading) and exercise intensity were not adequately described. #FallsPrevention #geriPT buff.ly/3uaimDB

12 weeks of balance training, 3 sessions per week, 36-40 sessions in total, 45 mins per session, and a total duration of 120 mins per week is the most effective in improving overall balance performance in healthy older adults. #FallsPrevention #gerotwitter buff.ly/3sSQFyh

Tbh, the strength of all lower limb muscles are important. Strengthen them all. Ie load bearing functional exercises

Perturbation-based training reduced laboratory falls from up to 56% to 0% after receiving 12 to 24 perturbations in older adults. One study also reported a 50% reduction in daily life falls at 12 months after a single perturbation session. #ReactiveBalance buff.ly/3LD6VtT

Waiting for the discharge to be safe❓
Waiting for the physio ax before 'allowing' the patient to walk❓
Waiting for the results of one more test before they can go home❓
If you wait for zero risk. The patient never stops waiting❗#EndPjparalysis

So many RCTs that include exercise programs include the same exercises daily or multiple times per week
The bodybuilders in the 70s knew that bodies need a REST! Hence split body part routines (granted intensity was generally higher)
More is not always better!
