Across 3 prospective studies, the signal for post-prostatectomy SBRT in 5 fx is increasingly consistent: feasible, low toxicity & excellent in-field control. StereoBed randomized data show no PRO/toxicity penalty; POPART/esSBRT show few local failures. #ASTRO26 @rtdess @HimanshuNagarMD @RadOncDocJ @NRGonc @ASTRO_org @urotoday @ChadTangMD @TylerSbrt @SbrtSean @jryckman3 @seanmmcbride

So proud of our #POPART around the 🌍 superbly presented by @PanizzaDenis 👏🏼 @StAr_MCM74 @valefacc22 @CChissotti @CiroFranzese1 @Raffa_Lucchini @roberta_muni
Post-prostatectomy RT in just 5 fractions? 👀 Ferrario et al and POPART team reports 100 pts treated with prostate-bed SBRT (32.5 Gy/5 fx): • Late ≥G2 GU toxicity: 8.5% • Only 1 late G2 GI event • Just 3 in-field relapses • 30-mo biochemical control: 74% Very encouraging local control and toxicity—but urinary QoL remains important to watch. #ASTRO26 #ProstateCancer #SBRT @HimanshuNagarMD @rtdess @RadOncDocJ @angela_jia_ @urotoday @TylerSbrt

And PACE-POSTOP coming: (66/33 or 52.5/20) va 32.5/5 Should be open early 2027…
Replying to @lauren_henke
SO, the GU-003 data could continue to look worse with time as f/u matures, because of the DOSE ESCALATION. 📉 These data don’t kill hypofrac, they just strengthen the case for 52.5 Gy/20 fx, as used in RADICALS, to be preferred hypofrac postop regimen—shorter tx **without the dose escalation** used in GU003. #ASTRO26 #ProstateCancer   #ASTRO26 #Pcsm #RadOnc

“How to’ do SBRT in prostate cancer” consensus recommendations is to define minimum requirements, common practices and additional options for prostate SBRT based on scientific evidence, expert opinion and consensus. @RO_GreenJournal @ESTRO_RT Read it 👉 thegreenjournal.com/article/…

Congrats on this work and publication @ZilliThomas ! Yours and @StAr_MCM74 show good tolerance with short to medium term FU. Maybe less hypoxia with SBRT will show that SBRT x1 is equally effective as SBRT x5. But need to do RCT. What would be your primary outcome?

Single-fraction SBRT for localized prostate cancer continues to push the boundaries. Multicenter phase 1/2 trial (45 pts), single 19-Gy fraction:
✅ 3-year bRFS: 92.9%
✅ Grade 2+ GU AE: <10%
✅ Grade 2+ GI AE: <5%

Great debate @ESTRO_RT - do we need augmented/adaptive radiotherapy for prostate cancer? @CZamboglou @DrAndrewLoblaw @valefacc22 and Adam Mitchell @royalmarsdenNHS Good points made by both sides, but if we can do better, surely we should?

First ESTRO missed since Madrid 2021…a lot of FOMO, but I’ll still be there in thoughts and somehow also in face 😊 Don’t miss tomorrow debate on augmented SBRT for prostate cancer, which I was honored to contribute to 🌰 Can’t wait to follow the discussion! #ESTRO26 #PCa #GU

📢 NRGGU005 Phase III: 698 pts with intermediate-risk #ProstateCancer RCT; SBRT (n=353) vs M-IMRT (n=345) ✅ SBRT → better bowel HRQOL (MCID 35% vs 44%, p=0.03) ⚖️No diff in urinary HRQOL; EPIC continence favours SBRT ❌futile for DFS; IMRT had ⬇️3-yr biochemical failure (4.2% vs 7.8%) SBRT safe, better QoL, ⁉️but not superior for DFS #ASTRO25 #RadOnc #PCSM

Negative trial -OS no difference -PCSM no difference -DM no difference -BCR was not SS different with only 4% difference at 10 years. -toxicity increased 3 cooperative group trials now negative: getug01, Rtog 9413, Rtog 0924.
