While I was on leave, I spent some time reflecting and writing about my experience with depression during pregnancy. A huge huge huge thank you to @awaisaftab for giving this piece a home! 1/x

🚨 I’m helping to lead more than 100 colleagues in a bipartisan effort for SNAP to allow hot food purchases. Hundreds of thousands of Virginians rely on SNAP — and working parents should be able to use their benefits to buy a hot sandwich or rotisserie chicken at the store.

Most common thing I say when deprescribing is "that was right for you x years ago, but you're not the same now as you were then so let's review this" #bgsconf

🚨 Elevated Prolactin with Antipsychotics CANNOT be Overlooked Here are some findings that should motivate psychiatrists to proactively treat ⬆️ Prolactin 1️⃣Females have a much higher risks of hyperprolactinemia related adverse effects 2️⃣Prolactin related effects are strongly associated with ⬆️ risk of suicide [Brugnoli R, et al 2012] 👉Amenorrhea ⬆️suicide risk by 64% 👉Galactorrhea ⬆️suicide risk by 163% 👉Gynecomastia ⬆️suicide risk by 200%. 3️⃣🚨Breast Cancer in women "Two recent epidemiological studies found an association between use of prolactin ⬆️ APs and breast cancer in women with schizophrenia and other psychotic disorders. In the two studies, estimated increased risks of breast cancer were disconcertingly high (up to 62%)." A 3rd study showed weak dose reponse association. [Hope et al, 2023] 4️⃣ Recent pharmacovigilance studies in humans (via FDA reporting system)👉a significantly greater proportion of pituitary tumours were reported with risperidone than with other available antipsychotics. 🚨 5️⃣Risperidone, Paliperidone and Amisulpride (higher doses) are most notorious. 6️⃣ Treating elevated prolactin is not complicated. Consider prolactin sparing agents - Aripiprazole (reverses it), Brexpiprazole, Cariprazine. [Quetiapine, OLZ , CLZ are PRL sparing but have metabolic effects] 7️⃣Metformin is also evidence based and shows good evidence of PRL reduction. "If they don't have any clincial symptoms there is no need to treat it" Should NOT be the norm. Below is an image of microdenoma of pituritary in a male in whom we identified a breast lump at prolactin at approx 1100mIU/L. 🚨🧠 Treat the patient not the level. #Psychtwitter #medtwitter

After a college student finally found a treatment that worked, the insurance giant decided it wouldn’t pay for the costly drugs. His fight to get coverage exposed the insurer’s hidden procedures for rejecting claims. propublica.org/article/unite…

Publicly shaming an Ivy league school for claiming credit for the success of a women scientist whose work it disrespected and rejected is one of the best uses of community notes I’ve ever seen.

Severe/moderately severe depression was associated with incidence of 29 nonoverlapping conditions requiring hospital treatment during a 5-year follow-up. Which ones? 👇1/ #Psychiatry pubmed.ncbi.nlm.nih.gov/3713… – at Melbourne, Victoria

New analysis updates ideal dose of omega-3 in #depression to 1,500mg/d. Also best results w/ EPA >= 2x DHA amount pubmed.ncbi.nlm.nih.gov/3772… Study finds omega-3s treat active depression, but do not prevent depression in the general population Good products: moodtreatmentcenter.com/prod…

Table 6 in this consensus guidance paper gives various treatment options for POTS and other autonomic disorders. Though the paper is on post-Covid autonomic dysfunction, treatment applies to autonomic disorders regardless of the etiology. onlinelibrary.wiley.com/doi/…

This is a good teaching case: pheochromocytoma is rare, POTS is common so think #POTS if you have a patient with this presentation. Importantly, don't think "anxiety" and send the patient to a psychiatrist or put them on an SSRI. Other things to think about besides POTS and pheo: mast cell reaction to a trigger (#MCAS), cardiac arrhythmia, pulmonary embolism, severe anemia, hypovolemia and carcinoid. #MedTwitter #MedEd

Fantastic thread.
🧵(1/n) An unfortunate example of confusion around the value of RCTs common to practitioners with limited research training. RCTs are a useful scientific method, but they are only a small part of the biomedical researcher's deep toolbox. xitter.cf/apsmunro/status/165554…

For years, I raised concern treating AFib detected by watches may not avert stroke/save lives. RCT out now shows anticoag after detection by implanted devices (more accurate & higher risk pop) leads to more bleeding without significant benefit. 1/ nejm.org/doi/full/10.1056/NE…
