Here’s a double-barrel STA-{M4+M2} bypass, performed after initial STA-M4 bypass for ICA stenosis precipitated ICA occlusion. The deeper bypass to M2 trunk injected robust flow w/ better redistribution into MCA territory than the more superficial STA-M4 bypass and saved the day.
