🦠 Gut microbiome composition is associated with response to neoadjuvant chemotherapy in pancreatic ductal adenocarcinoma patients New #GUTScience journal research by Halle-Smith et al. asks an important question: could the bacteria living in our gut be linked to how well pancreatic cancer responds to chemotherapy? The researchers found differences in the gut microbiome of patients who responded differently to neoadjuvant chemotherapy. Some of the differences involved bacterial genes linked to how medicines are processed. Why could this matter for clinicians and patients? 🩺 Pancreatic cancer can be difficult to treat, and patients can respond very differently to the same treatment. Understanding whether the gut microbiome is linked to treatment response could eventually help researchers develop better ways to predict — and potentially improve — how patients respond to therapy. It is still too early to use the microbiome to guide individual treatment, but these findings add to growing evidence that the microbes living in our gut could be relevant to cancer care. 📖 Read the paper: bit.ly/4f6XRzg #GUTScience #PancreaticCancer #CancerResearch #GutMicrobiome #Microbiome #Chemotherapy #PrecisionMedicine #TranslationalResearch @druppygill @Gut_BMJ @BMJOpen_Gastro @FrontGastro_BMJ

Gastrointestinal Stromal Tumor (GIST): Diagnostic Approach Gastrointestinal stromal tumor (GIST) is a mesenchymal neoplasm that predominantly arises in the gastrointestinal tract. Immunohistochemically, GISTs commonly express CD117 (KIT) and DOG1. Molecular testing demonstrates activating mutations in the KIT or PDGFRA genes in the majority of cases. A small subset harbors alternative molecular alterations, including mutations involving the succinate dehydrogenase (SDH) subunits, BRAF, NF1, KRAS/NRAS, and PIK3CA, as well as gene rearrangements involving NTRK3, FGFR1, and BRAF, among others.

The study is this month’s #GUTEditorsChoice and features on the #GUTFrontCover. Awoniyi et al. present “Dysbiotic microbiota trigger colitis-associated colorectal cancer and imprint a distinctive bile acid profile in a PSC-IBD model” via bit.ly/4yGOxJr PSC-associated IBD carries a particularly high risk of colorectal neoplasia. This study provides important mechanistic insight into how the gut microbiome may contribute to that risk. The authors demonstrate that dysbiotic microbiota can promote colitis-associated colorectal cancer while producing a distinctive bile-acid signature. These findings strengthen the emerging view that microbial communities and their metabolites can actively shape the intestinal environment in ways that influence carcinogenesis. 🎙️ It is also the subject of the September #GUTPodcast. ▶️ Listen on Apple Podcasts: apple.co/4re8PaL ▶️ Listen on Spotify: bit.ly/4gPU4HD ▶️ Listen on the Gut Podcast website: bit.ly/3UL1EL7 #GUTFrontCover #GUTCover #GUTPodcast

Autoimmune gastritis has traditionally been viewed mainly as a risk factor for gastric neuroendocrine tumours and non-cardia gastric adenocarcinoma. But could its cancer risk extend further? Read the #RecentAdvancesInClinicalPractice #GUTRecentAdvancesInClinicalPractice by Lenti et al. entitled “Autoimmune gastritis: a hidden gateway to cardia and oesophageal cancers beyond non-cardia gastric adenocarcinoma” via bit.ly/3UW32dV This review brings together emerging epidemiological and clinical evidence suggesting that autoimmune gastritis and pernicious anaemia may also be associated with malignancies involving the gastric cardia, gastro-oesophageal junction and oesophagus. The authors explore possible mechanisms including corpus-predominant atrophy, intestinal metaplasia and vitamin B12 deficiency, while highlighting important gaps in our current understanding. The review challenges us to think beyond the traditional boundaries of autoimmune gastritis and raises important questions about future risk stratification and surveillance. #AutoimmuneGastritis
