CRRT vs intermittent renal replacement trial stopped early for concern of signal of worse mortality in intermittent group. You might need to read the conclusion a couple times though "intermittent hemodialysis was not noninferior to CRRT"
Presented at #LIVES2026: An open-label, randomized, noninferiority trial showed that intermittent hemodialysis was not noninferior to continuous renal-replacement therapy in critically ill adults with acute kidney injury. Reviewed three recent randomized trials for long Covid. Two are positive -- exciting news! -- and one is difficult to talk about. Free-to-read link in replies. @NEJM @NEJMClinician

Important case - I caught a couple of these over the years labeled as sepsis or actively septic but then had some exposure - always makes me wonder if there are others that are missed.
ICU Stories: An 80 yo woman is about to go home when she suddenly develops profound shock SBP: 40s–50s mmHg Cold. Clammy. Diaphoretic. Weak The ECG is interpreted as: “Anterolateral and septal infarct” iv fluids + norepinephrine aren't working... What would you do next?

Long COVID RCT results (n=126) as presented @ISLCPAIS in abstract form: PSRT had statistically significant & clinically meaningful improvement of primary endpt (somatic symptoms, SSS-8) & secondary endpts of fatigue, physical function, cognitive function, pain, & shortness of breath compared to two different control groups (usual care & mindfulness-based stress reduction [MBSR]). 1/2

Odd: High-dose flu vs. flu vax RCT in NEJM where primary endpoint NOT analyzed (stated reason of event rate too low) yet the conclusions are "appeared to be fewer" hospitalizations. I've never seen this in a major clinical trial before - am I missing something? Any precedence?
In adults 65 to 79 years of age, there appeared to be fewer hospitalizations for influenza or pneumonia with high-dose influenza vaccine than with the standard dose, with a similar incidence of serious adverse events. Full GALFLU trial results and Research Summary: nej.md/46712T2

Hi All, Let's give Ari Moskowitz @Moskowcrit a follow as his account was hacked and this is new account @Moskowcrit Crit Care at Monte in NY and longtime CRS member and collaborator! @NickJohnsonMD
For my EM and critical care followers, Ari Moskowitz's @Moskowcrit X account was hacked and he had to get a new account going from 1K+ followers to 20s. His new account is: @Moskowcrit

CONTROVERSIAL TAKE: In 2005, ACLS guidelines made a dramatic change to solve a specific problem. But, the change was made without much data and should this be reconsidered 20 years later? 1/

KETAMINE VS. ETOMIDATE RCT (n=2,365). NO difference in 28-day mortality..But, ketamine worse secondary endpt of "cardiovascular collapse" 22% v. 17% though seemingly driven by hypotension as opposed to cardiac arrest (combined endpoint of SBP<65, new/incr vasopressor, CA).
Presented at CCR Down Under 2025: In a randomized trial involving critically ill patients undergoing intubation in EDs or ICUs, the use of ketamine for the induction of anesthesia did not lead to significantly lower in-hospital mortality than etomidate. Full trial results: nej.md/44EOlxB @CritCareReviews

Great job @MuhammadMu10165 on presenting his work on CoQ10 and Lipoid acid in cardiac arrest! Increasea oxygen consumption in-vitro but can we translate this to in-vivo and then someday to outcomes? More work to come!

NEW arterial line study which is portrayed as "arterial line" versus 'no arterial line" comparison for those who would typically get an a-line. But, this might NOT be the case and may not really be reflective of the study design! 🧵🧵 nejm.org/doi/full/10.1056/NE…

JAMA - large RCT of Bicarbonate infusion versus no infusion for metabolic acidosis in setting of acute kidney injury - no difference in 90-day mortality. Plan to look in more detail but bunch of interesting studies from ESICM this year.
For #criticallyill patients who had both severe metabolic acidemia and moderate to severe acute kidney injury, sodium bicarbonate therapy did not significantly decrease day 90 all-cause mortality. #LIVES2025 @ESICM ja.ma/4nsi8jE

I'll be doing the "con" of a debate at the AHA ReSS meeting in New Orleans on the broad topic of the new 2025 BLS/ACLS guidelines (link below). I have an idea of where I will go with this but want to hear yours - what controversies would you discuss? ahajournals.org/toc/circ/152…
