POCUS Cardiac Output Mental Math Hack: We all know that an incorrect LVOT diameter measurement gets squared, making POCUS-estimated cardiac output (CO) inaccurate. BUT assuming an average LVOT diameter of 2.0 cm, here is a quick mental math trick I use: CO ≈ 3 × LVOT VTI × HR 👉 Example: LVOT VTI 10 cm × HR 100 bpm ➡️ CO ≈ 3.0 L/min How formulas 1-4 (👇) collapse into this (👆) 1-step trick: 1️⃣ CO = SV × HR 2️⃣ SV = LVOT VTI × CSA 3️⃣ CSA = π × r² 4️⃣ CSA ≈ 3.0 × (LVOT Diameter / 2)² Why does this hold up clinically and mathematically? - 2D TTE (PLAX) may underestimate the minor axis of an ovoid LVOT. - 3D TTE & CT planimetry show true LVOT diameter skews larger (~2.6 cm). - π ≈ 3.0 keeps arithmetic lightning fast. Bonus: slightly underestimating CO is a safety feature and a good thing! On the other hand, overestimating CO creates a false sense of security! VTI alone works great (especially for trends), but when you need a numeric CO value FAST, use this shortcut! #FOAMed #FOAMcc #POCUS #CriticalCare

Thrilled to share results of the ICECAP trial, published in @JAMA_current: longer cooling durations did not improve neurological outcomes. Grateful to our team, colleagues, and most important patients/families who collectively made this trial possible. jamanetwork.com/journals/jam…

So excited! Sepsis Forum is  partnering with the Oregon (soon to be PacNW) SCCM. Panel on sepsis survivorship, a group discussing how to navigate the tension between guidelines & personalized sepsis care. Dr. Terri Hough @OHSUNews will be our keynote speaker on Sunday.
🚨 Registration is now open! Join leading clinicians, researchers, data scientists, and patient advocates for the 2026 Pacific Northwest Critical Care Medicine Symposium. Be a part of the conversation shaping the future of ICU and sepsis care! ➡️tinyurl.com/mt3sjcva

One of my favorite quick hemodynamic tools is the shock index: SI = HR/SBP Hypotension is a late finding and shock index detects early tachycardia and a softening of BP. Shock index has been most validated in trauma (looking for occult hemorrhage) but actually also predicts outcomes in pulmonary embolism, heart failure, and sepsis (although less precisely) On ShockCalcs, we've added a bunch of the evidence in the theory and practice section of the shock index calculator to check out 👇 Thresholds and interpretation in different populations also present on the page.

The best way to improve is not to focus on results. It’s to focus on habits. 27 studies: Behavior change goals are 3x more effective than outcome goals in boosting performance. Growth depends on altering the actions we can control. A key to progress is concentrating on process.

The #ANDROMEDA2 algorithm may look complex at first glance, but its physiology is beautifully simple. I’ve turned its #CRT-guided sequence into a pocket #checklist in an attempt to make bedside resuscitation easier, same science, another format. @AndromedaShock @edu_kattan

(1/3) The hemodynamic interfaces paradigm proposed by @ThinkingCC et al. is probably the most comprehensive paradigm for assessing shock. Not going to lie, its a bit complicated. I comprehensive calculator with theory and practice tips to help you understand the interfaces as part of a NEW project ShockCalcs👇
