Is autoimmune sclerosing cholangitis the same as PSC? Does overlap even exist? Thank you @JHepatology for accepting, and to @amandaricciuto for writing a stellar piece! @BinitaKamath @AutoImmuneLiver sciencedirect.com/science/ar… @jfergus75 @TheBileDoc @PSCSupportUK @PSCPartnersCa

🔥 What has changed in #diverticulitis management from 2018 → 2026? Less routine antibiotics, more individualized care, and important changes in recurrent disease and post-abscess management. 📚🔄 Great guideline update at #UEGWeek2026! 👏 @my_UEG #MedTwitter #Guidelines

The UEG Journal best paper prize has been awarded at #UEGWeek 2026 to Mathias Cook for his paper on Naldemedine for preventing recurrent acute pancreatitis. This research brings hope to many patients where this is no preventative options available. Congratulations 👏🏆 onlinelibrary.wiley.com/doi/…

🔬 Early treatment, lasting benefits. Presented at #UEGWeek, five-year follow-up of the PROFILE trial showed improved long-term outcomes with early top-down anti-TNF therapy versus conventional step-up treatment in newly diagnosed Crohn’s disease. See the findings at a glance!

In 2026, is it fair to compare ERCP with PTC, a procedure with significant morbidity, in unresectable hilar cholangiocarcinoma? The TESLA trial, presented at #UEGWeek today, didn’t evaluate EUS-guided biliary drainage either. Surely time to rethink the comparator? 🧐 @DrLakhtakia
The TESLA trial - my highlight at today’s late breaking abstract session at #UEGWeek Question: For malignant hilar obstruction, which is better: percutaneous drainage (PTC) or endoscopic drainage (ERCP)? Result: PTC > ERCP for unresectable hilar strictures.

What is the optimal duration of antibiotics after ERCP decompression for cholangitis? The #UEGWeek 2026 top abstract prize goes to Anouk Overdevest for her work on the COBRA trial, which shows that 1 day of antibiotic is non-inferior to 4-7 day courses 👏🏆

Gastric intestinal metaplasia (GIM) - pearls from #UEGWeek 💡 1. Differentiate autoimmune vs H pylori and biopsy using Sydney protocol 2. Risk stratify based on MAPS3 guidelines 3. Use OLGIM staging for extent 4. For autoimmune gastritis, consider immunology, biochemistry and 3 yearly surveillance

The UK’s current 30-day hospital readmission rate of roughly 15% represents a significant long-term upward trend, having nearly doubled over the past 30 years. This has coincided with hospital admissions falling from 8.5 to 4.5 days on average Also a 50% cut in hospital beds
Looking at guidance for people doing admissions and rounds in hospital. The majority of emphasis is on discharge. How about we concentrate on looking after patients properly in hospital rather than planning their discharge on the day they are admitted? We might prevent re-admissions and look after people properly?

Family is everything in recovery. Blood or otherwise. We do why we can with regards medicines, non-pharmacological interventions and surgery, but family is the fourth domain of therapy.
My patient was a metalhead biker with severe alcohol-related liver disease. Long hair, tattoos, piercings. “Who’s your family?” I asked. “My riding crew.” The next day, more than ten bikers filled our waiting room. And magics happened: But first, let me tell you how they got there. When he told me his crew was his family, I asked whether he would feel comfortable inviting them to discuss his illness and how they could help. He immediately picked up his phone and called a friend. Then he turned to me. “They’ll all be here tomorrow.” And they were. More than ten people, dressed in black, many with long hair, tattoos and piercings. I brought them into a separate room with my patient. I admit I felt a little intimidated. Then my patient asked me to speak openly. “Tell them honestly what I have. And how my crew can help me.” I explained that he had severe alcohol-associated hepatitis. That his life was at risk. That stopping drinking was essential, and their support could make an enormous difference to his recovery. Then they began to speak. They would do whatever it took to help their friend. They would never drink in front of him. They would make sure he always had someone beside him, so he felt supported, protected and accompanied throughout his recovery. Their voices broke. Many were in tears. So was I, almost. From that day on, someone from the crew was always at his bedside. After discharge, their support continued. He did not return to drinking. He recovered fully. I still think about that room. As doctors, we ask about “family support.” Sometimes, we need to ask who that family actually is. For this man, it was his riding crew. And when he needed them, they all showed up. That day reminded me how essential human connection can be in overcoming addiction—and how easily appearances can mislead us. Those metalheads were among the most empathetic, tender and committed people I have encountered as a doctor. Never judge a book by its cover.

If a GP (a highly qualified doctor) says a child has appendicitis, then it is for someone who actively manages appendicitis regularly, to say they do not, and no-one else. Simple. A correct diagnosis countermanded by someone less qualified is inexcusable.

Hepatology’s simplest equation: 1. 🍺 Alcohol-associated liver disease is the main killer. 2. 📈 Abstinence improves survival. Yet addiction training is missing from the curriculum. Addiction medicine must be core training. Lives depend on it.
