As a neurosurgeon married to a hepatobiliary surgeon, I have to strongly disagree with Dr Lawton. “Simple, consistent anatomy” it is not. If anything, with the long history of image guidance, I think quite a few neurosurgery procedures are ripe for robotic surgery. @HPBCambridge
Everybody’s talking robotics. But robots are built to do one task w/precision, need simple, consistent anatomy to operate surgically, like w/cholecystectomy. This brain AVM extends from cortex to ventricle, is supplied by convexity arteries + deep perforators, sits next to motor strip. Good luck Optimus….

In resected gallbladder cancer patients, age ≥75 was not associated with higher complications or worse recurrence-free survival, despite poorer overall survival. My view: age should inform risk, not define candidacy.

Excited to present the latest paper from the @HPB_OMEGA_study confirming that fitness matters more than age alone when considering resection for #gallbladdercancer @CUH_NHS @Cambridge_Uni @EAHPBA @IHPBA @CharityAMMF @curecc Huge effort from all our international collaborators 🙏
Age ≥75 years predicts poorer survival but not higher complication rates after curative gallbladder cancer resection
➡️doi.org/10.1093/bjsopen/zrag…
The prognostic influence of age on outcomes after gallbladder cancer (GBC) resection has been poorly characterised relative to other hepatopancreatobiliary malignancies. This international, multicentre retrospective cohort study analysed 3676 patients undergoing curative GBC resection across 133 centres in 41 countries (2010–2020), comparing outcomes in patients aged ≥75 years versus younger patients using propensity score matching. Older adults underwent less extensive surgery and lymphadenectomy and received adjuvant chemotherapy less frequently, and age ≥75 years was independently associated with poorer overall survival in both unmatched (HR 1.34) and matched (HR 1.31) cohorts, while recurrence-free survival and postoperative complication rates did not differ significantly by age group. The authors conclude that fitness rather than chronological age alone should guide eligibility for curative treatment, including major resection and adjuvant chemotherapy.

Mesopancreas excision ✂️ International consensus
🗺️ Defined as retropancreatic compartment of lymphatic neurovascular and adipose tissue
👍 Both artery- and mesenteric-first approach OK
🧼 Total excision more curative?! to be investigated further

Post-hepatectomy liver failure
➡️ doi.org/10.1093/bjs/znag004
🫀 PHLF remains the Achilles’ heel of liver surgery, with high mortality once severe liver failure develops
📊 Risk depends on functional liver remnant, liver quality, extent of resection, and postoperative complications
🔍 No reliable treatment exists, making preoperative assessment and prevention essential
📈 Volumetry, functional testing, and liver augmentation strategies are key before high-risk major hepatectomy
