This old thread tells the story of how television 📺 propagates the overweight and obesity stigma.
Unfortunately the 1000-Lb. Sisters gives a false impression on the experience of most people who undergo #bariatricsurgery. “Television” likes to promote extreme situations for entertainment purposes. It is NOT a fair representation of most #obesity RX. 1/ apple.news/ABoqZ5-AORNuWk1AX…
Hospitals are beginning to fire doctors and they are starting with the least lucrative: Pediatricians
Anyone not surprised pediatrics is the first to go? Silly employee docs, your heads are the next to roll.

Metabolic Bariatric Surgery results in 32% lower long term mortality compared to GLP-1
Metabolic surgery vs GLP-1 medication is a tradeoff: about 0.5% early mortality, but 32% lower long term mortality, around 12 life years gained per 100 patients, and fewer CV and renal events.

This is the next step in #AImedicine or #MEDAI
Microsoft's MAI-DxO paper is out in Science this week. On 304 hard NEJM case records, its AI "panel" reached the right diagnosis 83% of the time. Generalist physicians working alone, with reference tools, got 36%.
What interests me more is how it works. It isn't one model guessing. It simulates a group of doctors who propose, challenge each other and weigh each test against its cost. The benchmark scores the path, not just the answer.
That's a tumour board. For a hard gastric cancer case, I wouldn't want one surgeon deciding alone either.
So the fair next test isn't AI vs a lone generalist in a text-only setting (the paper says as much). It's AI panel vs human panel, then both together, on real patients.

Same body, different fat. In 42 women with severe obesity, fat under the skin and around organs had different gene activity. After bariatric surgery, some under-skin signals changed, but others still differed from controls. doi.org/10.1038/s41366-026-0…

Obesity is directly linked to an increased risk of several types of cancer. Treating obesity through METABOLIC BARIATRIC SURGERY can act as a protective factor against different forms of cancer, while also addressing the other conditions associated with the disease.

New research published in JAMA Surgery links bariatric surgery to a lower risk of obesity-related cancer.
The findings highlight the potential long-term health benefits of treating obesity beyond weight loss.

New in Obesity (Albaugh, Schauer et al., Pennington): 1.26 million US bariatric cases from the MBSAQIP registry, 2015 to 2023.
5.3% had a BMI of 60 or more. About 1.1% a year were 70 or more.
At BMI 70+ vs under 40:
Serious complications 2.6% vs 0.7%
Mortality 0.35% vs 0.05%
Functional dependence 4.5% vs 0.5%
The relative jump looks frightening. The absolute numbers are still low, and the authors conclude extreme BMI on its own shouldn't rule out treatment.
As a surgeon building a programme, my takeaway is that the operation is rarely the bottleneck. The clinic scale, the transfer, the table rating, the CT gantry and a recovery bed ready for sleep apnoea decide whether that patient ever reaches theatre.
One honest caveat: the registry only counts people who had surgery. Those turned away never show up in it.

Among individuals with a BMI of 35 or above, those who underwent bariatric surgery had a lower incidence of dementia and Parkinson's disease than those who did not undergo bariatric surgery. Bariatric surgery was also linked to a lower incidence of cardiovascular disease, obesity-associated cancers, and all-cause mortality. mdsc.pe/4s8F9MB

But, prevalence of Extreme Obesity (BMI >60) is increasing at alarming rate! pubmed.ncbi.nlm.nih.gov/4028… Obesity meds have only been tested for mean BMI 34.
