Iron helps make haemoglobin, produce energy and support cellular functions. But free iron can damage cells, so it needs safe transport. This is where transferrin comes in. Transferrin is a protein made mainly by the liver. It binds to iron, carries it through the bloodstream and delivers it to cells. What is transferrin saturation (TSAT)? Think of transferrin as a fleet of delivery vehicles. TSAT tells us how much of their iron-carrying capacity is being used. • Low TSAT: Limited iron availability. • Normal TSAT: Generally adequate availability. • High TSAT: May indicate iron overload. TSAT helps assess iron deficiency, anaemia and iron overload. What happens when iron is low? The liver may produce more transferrin to increase iron-carrying capacity. Low iron → More transferrin → Lower TSAT. However, inflammation, liver health and nutritional status can also affect transferrin levels. Ferritin reflects iron stores, while transferrin and TSAT help assess iron transport and availability. Doctors interpret these alongside haemoglobin and serum iron. How can you improve iron absorption? 1. Include iron-rich foods: Dals, beans, rajma, chole, tofu, seeds, ragi, leafy greens, fortified cereals, meat, fish and eggs. 2. Pair plant-based iron with vitamin C. Try dal with lemon or iron-rich meals with guava. 3. Have tea and coffee between meals to reduce their effect on iron absorption. 4. Soak, sprout and ferment foods to help reduce phytates. 5. Eat varied iron-rich foods rather than relying only on spinach. 6. If ferritin is low, seek medical advice. Supplementation and investigation into the cause may be necessary. Avoid high-dose iron without guidance. Transferrin delivers iron where it's needed.
