This was indeed malakoplakia. RDD a good thought too.
Lymphoid cells were B-cell rich, but contained reactive germinal centres, and interpreted as reactive.

Kidney can also have lots of different tumors. Female, 45yo with a metanephric adenoma!. Stains are PAX8 and WT1

This came to me as negative for malignant cells from the cytotech. These deceptively bland cells are all basically all malignant and BER-EP4 positive. Take a guess about their origin. #boardfodder

Basal cell adenoma is a benign salivary gland tumor, mainly of the parotid gland, with biphasic basaloid and ductal cells. It shows varied growth patterns, peripheral palisading, and nuclear β-catenin positivity at the periphery (CTNNB1 mutations). #pathology #pathtweet

UGLY NUCLEI. BEAUTIFUL CLEAR CELLS. A TERRIFYING DIAGNOSTIC TRAP! 🔬
A 31-year-old woman. 27 weeks pregnant. A massive 12.5 cm ovarian cyst.
Under the microscope?
Bizarre, pleomorphic nuclei. Striking clear-cell cytoplasm. Cells that could make you fear ovarian clear cell carcinoma!
But the diagnosis?
Large solitary luteinized follicle cyst of pregnancy.
Completely BENIGN!
Pregnancy-related hormonal changes can produce astonishing cellular appearances that mimic malignancy.
A wrong diagnosis could mean unnecessary radical surgery and devastating consequences for a young mother.
The lesson?
Never let ugly nuclei dictate the diagnosis.
Never interpret high-power morphology without low-power architecture.
And NEVER underestimate clinical history.
Sometimes the most frightening cells are completely innocent.
