🦶 Therman Test of the Ankle The Therman Test helps assess subtalar instability in patients with recurrent ankle sprains or a feeling of the hindfoot “giving way.” With the patient supine and the ankle in approximately 10° of dorsiflexion, stabilize the forefoot and apply inversion and internal rotation to the calcaneus. Excessive medial shifting with reproduction of pain or instability suggests a positive test. 🔗 Link: wikism.org/Therman_Test_of_t… #SportsMedicine #AnkleInstability #PhysicalExam #Musculoskeletal #MedicalEducation

When plan A shifts, it’s time to improvise. 🔄 On MRI, the retrotalar sheath of the flexor hallucis longus (FHL) was distended with fluid. An easy injection, right? 🥄 But the MRI was performed with the patient supine, while the procedure was done prone. With the change in position, the fluid shifted into the plantar extension of the sheath, leaving the retrotalar segment collapsed. The solution? Compress the plantar aspect of the foot to move the fluid back. To maintain pressure during the injection, I taped a cotton ball in place to take the job of my finger. The fluid shifted back into the retrotalar sheath, and the injection became straightforward again. 👣💉 A simple, practical trick when fluid shifts within a tendon sheath with patient positioning. Ultrasound-guided procedures often call for anatomical insight, a little creativity—and sometimes an improvised extra hand. 🧠✨ #MusculoskeletalRadiology #MSKUltrasound #UltrasoundGuided #UltrasoundIntervention #FHL #FlexorHallucisLongus #TendonSheath #SportsMedicine #Radiology #MedicalEducation

The foot moves. The range is restricted. In this calcaneonavicular coalition case, subtalar motion was preserved; midtarsal motion was restricted. Assess each separately. Normal-motion clip: a different patient. Golshteyn & Schneider. Clin Podiatr Med Surg. 2022.

The shoe removal test evaluates for hamstring strains. The patient removes the contralateral shoe by externally rotating the affected leg to reach the opposite heel, mimicking normal shoe removal while standing. The test is positive when the maneuver reproduces the patient’s pain. In one study, sensitivity and specificity were 100%. Before ordering a scan or blood test, doctors still look at the nails. Cancer, lung disease, severe iron deficiency, heart infection and kidney disease can all leave clues there. Most changes are harmless. These five deserve a closer look: 1. A DARK LINE WIDENS

A 40-year-old patient presents with a chronic, progressive swelling of the foot and ankle, multiple firm nodules, draining sinuses, hyperkeratosis, and progressive architectural distortion. Granular material expressed from the sinuses shows both black and yellow grains. The classic triad of tumefaction, multiple draining sinuses, and granular discharge, together with the characteristic grain colors, is highly suggestive of mycetoma (Madura foot). 🔬 Question: What is the most likely diagnosis, and what do the black and yellow grains suggest regarding the possible etiologic organisms?
