We are very proud to have published Mastering Pediatric Spinal Deformity with @SpringerNature This is a two-year effort with @rajiviyermd, @mgv16 and @LLenkeMD
We are grateful to have more than 75 leading experts as chapter authors, all of whom are actively driving a specific part of the field forward. This 428-page volume delivers practical, actionable information on exactly how they are performing these procedures and thinking about pediatric pathology.
When Raj and I were fellows arriving at Columbia, this is precisely the kind of practical reference we were searching for. It is our sincere hope that surgeons at every level of training, in communities around the world, can take the expertise shared in these pages and bring it directly into their own practice.

You can change the name, but the same shady insurance company tactics persist.
A New York neurosurgery practice has filed a complaint in federal court challenging Elevance Health's policy of penalizing hospitals for working with out-of-network physicians.... even those complying with the No Surprises Act.
For those unfamiliar, @ElevanceHealth was formerly Anthem, which faced years of lawsuits, regulatory scrutiny, and controversy before rebranding in 2022.
It ranks #18 on the Fortune 500 list.
If insurers offer rates that don't cover the cost of delivering care, and hospitals are penalized for working with out-of-network physicians, how does that protect patient access or choice? Who has the leverage here? Who is getting squeezed? Who is contributing to consolidation? 👏Follow 👏the 👏money.

ICYMI: Addressing neurosurgery workforce shortages requires policies that expand access without compromising physician-led care.
In Forbes, @RMengerMD, highlights how APPs can strengthen neurosurgical teams, extend capacity and improve care delivery- while neurosurgeons retain responsibility for surgical decisions and overall patient care.

Appreciate the ability to contribute in an interview/quote for this incredibly impactful article by @SavannahTF . BCBS controls the insurance market in Alabama. This challenges core free market principles. If @BCBSAL doesn’t cover a certain procedure it’s extremely unlikely to occur. Coverage does not always equate with access. Great read. Nidus for change.

A single insurance company dominates Alabama. Doctors, patients say the impact can be ‘devastating’ mrf.lu/2dhFH
Excellent work serving @GreaterMobileAL by @jaithakurMD
Proud to be part of the department that can offer this type of care to the community

Neurosurgery awareness month!
Such a pleasure taking care of Ms. Laura. She got radiation(SRS) at another University Hospital but unfortunately her pain returned back very fast. Trigeminal neuralgia is so debilitating and is called suicide disease for a reason. So glad we could help her. We truly appreciate Laura and her husband for sharing their journey with everyone. @UofSouthAlabama

It’s neurosurgery awareness month and it’s important to highlight all the people who make neurosurgery possible. This is inside and outside the operating room.
Thanks to @KatieOrrico for sharing.
Tremendous work by the @AANSNeuro focusing this month on the precision neurosurgery requires.

Are unions for physicians a good thing? Why is it happening?
California’s 10,000 University of California attending physicians are organizing what could become the largest employed-physician union in the country.
It is the predictable endpoint of a multi-decade policy architecture that systematically converted independent professionals into hospital employees.
Medicare’s site-of-service differential, lagging physician fee schedules under budget neutrality, exploding regulatory and prior-authorization burdens, and the Affordable Care Act’s freeze on physician-owned hospitals all pushed practices into system employment. Once physicians lose residual ownership and clinical autonomy, industrial-style collective bargaining becomes the next logical (and second-best) response.
The risk is real: unions optimized for the median member compress skill and productivity differentials that matter most in complex procedural fields. High-skill specialists face flatter compensation structures, reduced operational flexibility, and weaker incentives to remain in high-volume academic settings.
Unionization addresses symptoms. It does not restore the residual rights, ownership stakes, or professional autonomy that made medicine a profession rather than a trade.

The Marines taught me one rule: the man at the top owns everything that happens below him. No exceptions.
Then I got into healthcare.
Where a CEO can gut a town's only maternity ward, miss every quality metric, and collect a retention bonus.
Two different definitions of leadership.
One of them I'd let near my kids.

🚨 REPORT: 82% of physicians are now employed by hospitals or corporate entities. Nearly two-thirds of practices are no longer physician-owned.
This outcome should not surprise anyone.
For more than a decade, Medicare payment cuts, mounting administrative burden, growing insurer oligopolies, and policies that disadvantage independent practices have made this outcome entirely predictable.
If lawmakers want more competition, lower costs, and better access to care, preserving independent practice must be part of the conversation.
No single reform will reverse decades of consolidation. But policymakers can start by fixing Medicare, fixing prior authorization, expanding physician-owned hospitals, and strengthening antitrust enforcement.

Thanks @jeffreymullinmd and @EladLevyMD . My latest in @Forbes goes after the idea of what surgeries are safe for an ASC? Historically neurosurgery has been cautious to enter into the arena but at @UB_Neurosurgery complex cranial and spine cases are being performed safely in the outpatient setting.
What does this mean for patients? What does this mean for surgeons? More importantly, how can this help inject some market-based competition into the delivery of neurosurgery?
Another great piece on the future of healthcare by the preeminent @RMengerMD

ICYMI: Some insurers are denying implants commonly used in neck/spine surgery by labeling them “experimental” — despite decades of use and broad clinical support.
Washington Committee member Luis Manuel Tumialán, MD, and spine surgery leaders are calling for coverage policies grounded in evidence-based medicine and clinical expertise — not outdated insurer guidelines.

Neo-feudalism?
Not just for your high school history course. Welcome to healthcare in 2026.
Physicians have been forced by regulatory crony capitalism to abandoned private property and become serfs to the feudal lords that are the expanding nonprofit healthcare systems. At best they will be granted a fiefdom. A title, a domain, but not ownership. And it’s a title that can quickly be taken away.
Modern healthcare has degraded to a medieval relic.
There are real changes that can fix this. My latest in @aier

Government-created healthcare monopolies are forcing physicians into a kind of serfdom, gobbling up private practices to extract doctors’ labor.

The public is trained to hear “hospital closing” and assume virtue.
Poor patients.
Bad reimbursement.
Heroic administrators.
Cruel politicians.
Sometimes true.
Often incomplete.
The harder truth is that the majority of hospitals are fragile because they have no real financial architecture, no coherent strategy, no physician alignment, and no one inside the building who knows how to run it.
That is fixable.
But not by pretending every failing institution is well-run.

Obviously management consultants don’t improve hospitals. Something everyone - including the consults themselves - already knew.
The purpose of these reports is to justify administration decisions. When something doesn’t work they point to the McKinsey/Bain/BCG report as cover.
US nonprofit hospitals spent $7.8 billion on management consultants from 2009 to 2023, but contracts were not associated with meaningful changes in finance, operations, or quality of care. 🧵
