After finishing my @UTMDAnderson residency, I've been reflecting on what I wish someone had told me as a new PGY2.
These are the five lessons I'll carry into faculty—and the ones I'll share with every future resident. #MedEd #RadOnc

Thrilled to share updated results from JNJ-6143, the AR-BRD4 RIPTAC therapy, presented by @BGarmezy. Totally novel MOA was well tolerated, oral pill and produced significant benefit for pts (PSA90 50%, ORR 68%). P2 trial active now and more coming. Lots of potential!

A benefit of working with @PavlosMsaouel is that we treat many variant RCCs. Renal Medullary Carcinoma is one such variant (0.5% of RCC), which has remarkably different RT sensitivity. MDT to 30gy/5 often achieves a CR and with minimal toxicity even with high dose chemo.
1/5 New in @EurUrolOncol: the largest series yet of metastasis-directed radiation therapy in SMARCB1-deficient #RenalMedullaryCarcinoma. Key finding that this cancer appears far more radiosensitive than clear cell kidney cancer: authors.elsevier.com/c/1n~As… #endRMC #MsaouelLab

🩺 The evolving role of SBRT in metastatic kidney cancer
At the 2nd Florence International Consensus, Prof. Chad Tang from @UTMDAnderson highlights how stereotactic body radiotherapy (SBRT) is becoming an increasingly important part of metastatic renal cell carcinoma (RCC) management. #FlorenceConsensus26
🎯 Oligometastatic disease SBRT may help defer systemic therapy and its associated side effects in selected patients. Local treatment approaches—including radiotherapy, surgery and interventional ablation—offer options that work through mechanisms distinct from systemic therapy.
🔬 Oligoprogressive disease By targeting a limited number of progressing lesions, SBRT may allow patients to remain on their current systemic treatment for longer, potentially delaying the need to switch to subsequent lines of therapy.
💡 The key takeaway: Across the metastatic disease spectrum, SBRT is expanding the role of local treatment in kidney cancer care, supporting more tailored treatment strategies and helping clinicians consider how best to manage disease progression while preserving treatment options.
📍 As evidence and clinical experience continue to evolve, multidisciplinary decision-making remains essential to identifying the patients most likely to benefit.

Alternatively, Pope Leo could have stereotactic ablative radiotherapy if his tumor is small, peripherally located, medium/high risk or refusal for lobectomy. 2025 ASTRO presentation comparing SABR vs VATS showed no inferiority in overall survival at 10 years. Manuscript will be published soon.
Pope Leo XIV has been diagnosed with lung adenocarcinoma.
While we had hoped for a different diagnosis, the fact that he was able to undergo a lobectomy is encouraging. Surgery is generally pursued when lung cancer appears localized and potentially curable.
Final pathology will help determine prognosis and whether additional treatment is needed.
Wishing the Holy Father a smooth and speedy recovery.

This is a tragic story that must be shared with younger generations. I was blessed to learn a similar confession early in my career which led me to pursue a more fulfilling journey, full of friendship and adventure. Yet, it hasn’t been easy. Fostering a fulfilling life requires a daily commitment to actively prioritize what matters most, and remembering to say “yes” to ourselves more often. @NehaVapiwala @CBaffic

Hi. Unfortunately the evidence base for this modality does not meet standards. There are no adequately controlled studies. OA is very unique in that the placebo effect is huge (40% pain improvement over 2 years in the GAIT-2 trial, for example). You need a good quality placebo controlled trial to establish efficacy.
Patients feeling improvement does not mean is not a placebo. Patients with OA feel improvement with PRP injectione, stem cell treatments and other things that do not beat placebo in a trial.
Its unfortunate for the patients but the evidence is not there to justify coverage at this time.

@ASTRO_org Not trying to be controversial or contrarian, but I think the decision to move from an actual, interactive poster session to digital posters has missed the mark. #ASTRO26
Attendance is down.
Exchange of ideas is down.
And most importantly, opportunities for trainees to interact with investigators and others in the field are down.
Poster sessions have always been about far more than displaying data—they create conversations, connections, mentorship, and collaborations, especially for our trainees.
It’s not too late to fix this. Let’s bring back meaningful, in-person poster interaction.
Congratulations @ChelseaPinnix!!!! 👏👏👏
