So... Jax didn't want to be hurt by people he cares about suffering, so to get around that... he makes everyone suffer by viewing them as cartoon characters.

But as soon as he realized it did matter what he did because everything was in fact real... he had a panic attack. Because deep down, Jax is torn by the thought of someone he views as human suffering.

Jax did kind of "want everyone to suffer", but in a way that doesn't matter ultimately. These people weren't humans in his eyes, so he could do whatever he wanted to them without having to feel bad about it.
