Can reopening a chronic total occlusion (#CTO) make the heart electrically less prone to #ventricular #arrhythmias?
For years, small studies suggested the answer was yes.
We analyzed 7,496 ECGs from 1,232 patients undergoing 1,520 CTO PCIs using automated Python-based signal analysis.
Here's what we found... 🧵

Do the atria matter? Yes! In this new study in Circ EP, we show that both R & L atrial measures are associated with ↑ risk of cardiovascular disease & provide genetic insights into atrial remodeling. #Cardiology @UWMedHeart @uwmnewsroom @UWDeptMedicine ahajournals.org/doi/10.1161/…

By applying deep learning analysis to cardiac magnetic resonance imaging, heart study @UWMedHeart advances understanding of role of both left- & right-sided atrial dysfunction in developing clinical complications like A-fib, stroke & heart failure. Women are at higher risk. Study also found genetic correlations for chamber-specific remodeling.

🫀 Same-Day Discharge (SDD) after CTO PCI — is it safe?
In our study published in @AmJCardio , we analyzed 1,273 consecutive CTO PCI cases (2019–2023) to evaluate the safety of SDD in contemporary practice. @LAzzaliniMD @UWMedHeart
📈 SDD increased from 11.7% → 44.3% over 5 years.
But what about outcomes? 👇

Another contribution from our Protamine Research Program @UWMedHeart
Protamine after #CTO #PCI complicated by coronary perforation is safe but did not lead to improved clinical outcomes, highlighting the importance of a multi-prong approach to the problem.
Should protamine be routine—or selective—when managing coronary perforation during CTO PCI?

There aren’t a lot of opportunities for “preventive medicine” in EP or VT, but our “prophylactic” approach for our Tetralogy of Fallot patients appears to have an early signal of safety and reduction in incident ventricular arrhythmias long-term (with fewer ICD implants)
