This year, TCT is stopping in San Diego (and I’ll have to take three flights to get there), and the late-breaking lineup is quite interesting. Here are my favourites, both journalistically and from an interventional cardiology perspective:
1/ P4, a trial of CCTA-guided vs IVUS-guided PCI. We published the design paper in EuroIntervention. Check it out: eurointervention.pcronline.c…
2/ TRANSFORM II, or DCB strategy vs DES, Act III. With REC-CAGEFREE I coming out negative and SELUTION DeNovo being so-so, we really needed a tiebreaker. And here it is.
3/ HD-PCI, a randomized trial of higher vs lower-dose heparin for PCI. I like trials that answer practical, “in or out” questions. I also have to discuss this one right after the presentation, so I’m studying it carefully.
4/ PROGRESS: TAVR or clinical surveillance for moderate aortic stenosis. I still haven’t quite figured out the ambition and magnitude of these TAVR trials in moderate AS. The first one, TAVR UNLOAD, didn’t leave much of a mark. We don’t know when EXPAND TAVR II will be presented, but I suspect it’ll be soon. And then there’s PROGRESS, the largest of them all in terms of patient numbers. By the way, ClinicalTrials.gov says follow-up will end in 2037. As we say back home, campa cavallo.
5/ SIMPLAAFY, antithrombotic monotherapy after left atrial appendage occlusion. I like this trial if only for its name, and I can’t wait to hear how people pronounce it. More importantly, it could give DAPT a serious shove out the door, since it seems to be the main burden holding back LAAO at the moment.
6/ PIONEER IV, angiography-derived FFR vs usual care for lesion selection and optimization in all-comers PCI. Another QFR trial occupying an intermediate position on the physiology spectrum between FAVOR III Europe and FAVOR III China.
7/ VERONICA, where researchers are trying to demonstrate the usefulness of platelet-function testing to identify patients with excessive platelet inhibition (usually, it’s the opposite) and see whether a de-escalation strategy could reduce the combined risk of ischemic and bleeding events.

AngioVAC for left-sided intracardiac masses shows good effectiveness. Careful case selection, timely workup, and optimal embolic protection strategy are necessary. @PedroMDMSc @ShimamuraMD @DrMarvinEng @AhmadJabri8 @g_giustinoMD @vonBardelebenRS @engelpedro @Tiberio_Frisoli
Transcatheter vacuum-assisted extraction of left heart and aortic arch masses showed promising early effectiveness in this multicentre registry.
The observed rate of adverse events suggests the need for careful patient selection and improved procedural safety.

Meta-analyses of intravascular imaging guidance of PCI show markedly improved outcomes, but only if you use this tool properly — pre-, intra- and post-PCI and with appropriate targets. How do you do it? Our new state-of-the-art review in EHJ takes you through it step by step.
Can someone explain to me why a professional football player was hospitalized for a suspected pulmonary embolus then was told not to sleep for about 36 hours ?
I pray with all my heart that the turmoil & violent demonstrations across Europe come to a peaceful halt as nobody wins , nobody benefits , nothing gets better , only worse !

As If Israel Didn't Have Enough Problems: Why Picking a Fight with Egypt Is a Strategic Mistake
The obsession of some Israeli ministers and politicians with confronting Egypt is not only counterproductive but also fundamentally at odds with Israel's national security interests.
Those attempting to portray Egypt as an enemy of Israel should answer a simple question: Do they seriously want Israel to rebuild its Southern Command's military capabilities for a potential confrontation with Egypt? Are they suggesting that Israel should once again prepare for war on its southern front? And with what resources, exactly?
Fifty-three years after the last major war between Israel and Egypt, there seems to be a troubling loss of historical memory regarding what relations between the two countries looked like before the historic peace agreement.
The peace treaty with Egypt is one of the most important strategic assets Israel possesses. Preserving it is arguably even more important to Israel's national security than expanding the Abraham Accords, including normalization with Saudi Arabia, as significant and desirable as that would be.
This does not mean ignoring Egyptian actions that may violate the peace treaty. Any such concerns must be addressed seriously through direct dialogue between Jerusalem and Cairo, with Washington playing an important role when necessary.
But Israeli politicians must stop treating Egypt as an adversary, stop making empty threats, and stop fueling tensions that are already dangerously high.
Israel has more than enough security challenges. Turning its most important Arab peace partner into another potential front would be an extraordinary strategic mistake.
The peace with Egypt should never be taken for granted. It took enormous political courage to achieve it, and preserving it requires strategic responsibility, not political grandstanding.

🇪🇬🇮🇱 Egyptian President Abdel Fattah el-Sisi responds to Israeli Finance Minister Bezalel Smotrich’s remarks that Egypt would not go to war for fear of losing Sinai:
“Who said we wanted to go to war? We have no agenda against anyone. We fought to get our land back, and we did. That’s settled. Are you still gripped by the same paranoia from 60 years ago? Do you still think we want your land? My only goal is to protect my country, its borders and its territory. As long as we respect each other’s territory, Sinai is ours and our borders are secure, we have no hidden agenda against anyone.”

In the late 1980s Ahmed Zewail developed methods for studying chemical reactions in detail. Chemical reactions in which molecules held together by atoms meet and reorganise into new compounds are one of nature's most fundamental processes.
However this transition happens too quickly for us to easily see - in a matter of femtoseconds. One femtosecond is 0.000000000000001 seconds, which is to a second as a second is to 32 million years.
By using laser technology to produce flashes of light just a few femtoseconds long, Zewail was able to visualise these reactions in slow motion - and to see what actually happens when chemical bonds break and new ones are created.
Zewail's experiments led to the birth of the research area called femtochemistry, which enables us to understand why certain chemical reactions take place but not others.

In a few hours we will be revealing the 2026 Nobel Prize in Chemistry - stay tuned.

Our work from PREVENT now publ in Circ. Only 37% of FFR neg severe lsns (PB>70% & MLA<4mm2) were lipid-rich plaques (LRP) by NIRS. Event rates were much higher and preventive PCI was only effective in LRPs. PCI of "true vulnerable plaques" may improve prognosis and avoid over-Rx.
