閉館された、シネスイッチ銀座。
映画祭の会場になっていますので、閉館前に最後に行けなかった方、閉館の情報を知らなかった方も、この機会なら。
編集部も含め、最後の姿を写真に撮るファンの方も多いですね。お世話になりました。

年末に近づくと翌年の大河ドラマ関連本が書店を賑わす。「逆賊の幕臣」の小栗上野介は滅びゆく江戸幕府の屋台骨を支えようとした悲劇の忠臣。
著者は物語に妻・道姫との絆を添わせ一味違う忠順像を作り上げました。

#ESMO26 is a wrap. A few thoughts.
It is a privilege to be an oncologist. To everyone who took the time to meet with me, lend their expertise or mentor me in some way: thank you. I am grateful. Also, to the patients who contributed their time, hope and lives to allow these studies to happen, thank you. This is what allows progress.
1. ESMO absolutely brought incredible advancements. So did China. This was the first global conference where the USA has clearly been dethroned as the de facto biomedical trial leader (5 of the 7 Presidential Symposium late-breakers focused on new drugs were Chinese developed). Chinese patients will be getting access to the latest biomedical advances, first.
2. The molecular era is here for KRAS. Expect it to come to early lines, in every tumor, fast. Expect sexy combos. (I am personally very excited about Genmab + Incyte's LGR5-EGFR + G12D but also think Verastem’s and RevMed’s G12D’s look great.)
3. Some advances look meaningful but are minor. PD1-VEGF beats PD1, but it wasn't compared to PD1 + VEGF. The benefit looks similar to adding bev (duh).
4. Cellular therapy is finally here for solid tumors. Get your systems ready now, it takes years to truly prepare. The 66% response rate for TezR (SL-28) across all-comer solid tumors blew my mind.
5. ADCs will hit CRC/GI quickly, and in a few years there will be many questions about sequencing. Please please please stop using the same payloads.
6. Medical education is more important than ever with how fast the field is moving. If anyone has advice on how @TheGutOncLab can disseminate information better (like the people at @OpenMedicineHQ and @OncoAlert do), please let me know.

5 major things just happened in medicine this week:
1. A breast cancer drug more than doubled the time before the cancer started growing again in a Phase 3 trial (16.4 mo vs 6.8 mo).
2. The world’s deadliest cancer (lung) was treated with a new therapy that cut the risk of death by 58% in a Phase 3 trial.
3. Chronic Hepatitis B now has a functional cure for certain patients after accelerated FDA-approval.
4. Retatrutide put up huge numbers at a sleep medicine conference showing it cut the amount of sleep apnea events in half on the highest doses, in a Phase 3 trial.
5. The leading cause of cancer death under 50 (colorectal, here specifically rectal) now has an FDA approved therapy allowing certain patients to skip chemo, radiation, and surgery.

以前、中国の薬剤を海外に流通してる会社の方からお話を聞いた事があったけど最近の中国新薬の勢いがヤバそうだね。
今回のESMOでも猛威をふるっている。
乳がんのizalontamab brengitecanもヤバそう。
何だよHR0.41って。
ただ、中国の薬剤って治験薬でも普通に裏市場で流通しているという噂も聞くので凄い世界だなと思う。
中国で治験を受けられたがん患者さんとYouTubeで対談動画上げる予定なのでまた報告します。
私も衝撃的でした。
患者さんが望めば中国への治験リクルートや薬剤の調達なども検討せねばならない時代なのかもしれません。
この対談は凄く面白いと思うから全Oncologist観て欲しい。
マジで世界が変わると思う。

2 years ago, the most common KRAS mutation in cancer had no drug at all. At this ESMO meeting in Madrid, 4 companies are presenting one.
I said it many times: KRAS is the accelerator pedal in a cell. Jammed on, the cell grows without permission, it drives a large share of colorectal, lung and pancreatic cancers. The most common variant is called G12D.
The drug presented today, QTX3034, is the latest one, it doesn’t even have a proper name yet. But in 36 patients whose colorectal cancer had already outrun standard chemo, about a quarter saw tumors shrink and roughly eight in ten had disease stop growing.
Early phase, small, no survival data yet.
But look at the field's shape: 4 drugs, same mutation, all reporting within days, the next one tomorrow with more robust data. That doesn't happen while something is still hard. It happens right after a wall comes down.
One detail, which always sticks out to me: median age 51. Youngest, 29.
That's young-onset bowel cancer from the inside, people in their 30s, already out of standard options, waiting for exactly this.

この日、神保町ブックフェスは雨で中止。イベントを見てもらうために自由時間を２時間取ったそうですが、残念でした。
毎年参加希望者が増加していて、今年は100名募集よりだいぶ多くて、書類選考があったとか。

I can’t kick off my ESMO meeting coverage without mentioning oncology’s star of the year 2026: daraxonrasib.
New data from Madrid today, and one number stands out. We already know that daraxonrasib doubles survival in previously treated metastatic pancreatic cancer. That's why it was approved in August.
Today we looked under the hood of the KRAS gene and which exact KRAS mutation each patient had. Because "KRAS mutation" isn't one thing: the gene can be misspelled in different ways. G12D. G12V. G12R. Same gene, different letter, potential implications for the patient?
First good news: The drug helped every group. But patients with G12V did clearly best, a 68% reduction in risk of death, versus 51% for the most common G12D and 40% overall.
I have to stress that these are small patient numbers and the trial wasn’t set up to show superiority between each mutation, but it points somewhere.
"You have a KRAS-mutant pancreatic cancer" may soon be too vague. The next question becomes which KRAS, because that might predict how well this works for you, or if you need a specific KRAS-G12D-inhibitor alongside, while daraxonrasib covers the rest of the gene.
And this combination is no pipe dream, there is already a large trial running where daraxonrasib teams up with the KRAS-G12D-inhibitor zoldonrasib. Do the 2 together work even better? Results to be expected next year.
Precision oncology is getting more precise. Not one disease, not one mutation. One letter might now make the difference. Exciting times in pancreatic cancer care.

個別化mRNAがんワクチン
「intismeran autogene」
の第Ⅲ相試験、INTerpath-001ですね。
すでにデータが世に出てしまったため、ESMOが正式に情報解禁を前倒ししました。
これ、何をやっているかというと、、
患者さんのがんの遺伝子情報を調べて、その人のがんに特有の目印を狙うmRNAワクチンを作る。
それを使って、免疫に
「この目印のあるがん細胞を攻撃してくれ」
と教える治療です。
患者さんごとに作る、オーダーメイドのがんワクチンですね。
今回の対象は完全切除後のStage IIB〜IVの悪性黒色腫の患者さん1,137人。
①個別化mRNAがんワクチン＋キイトルーダ（Pembrolizumab）
②キイトルーダ単独
を比較しています。
結果は、、
【2年無再発生存率】
82.2％ vs 70.0％ HR 0.58、p＜0.001
再発または死亡のリスクが42％低下。
【2年遠隔転移無生存率】
88.5％ vs 80.2％ HR 0.62、p＝0.002
遠隔転移または死亡のリスクが38％低下。
Grade 3〜5の治療関連有害事象は 14.2％ vs 13.4％でした。
全生存期間はまだ追跡が必要ですが、キイトルーダに上乗せして再発を減らしたというのは、臨床的には重要な結果だと思います。
ところが、、
モデルナの株価は、データ公開後に25％超下落したと報じられています。
「え、効いてるのに何で？」
ってなりますよね。
そもそもモデルナ株は、今回の治療への期待などから年初来500％超も上昇していました。
なので、良い結果が出ること自体は、すでに株価に織り込まれていたという話です。
加えて、先行する第Ⅱ相試験から期待されていたほどには
遠隔転移を抑える効果が大きくなかった、という受け止めもあったようです。
ただし、対象や追跡期間の違う試験をそのまま比較して「効果が落ちた」とは言えません。
個人的には、患者さんごとに作るmRNAがんワクチンが、第Ⅲ相で再発抑制効果を示したこと自体が大きいと思います。
ただ、これで色々ながんに効くと決まったわけではありません。
まずはメラノーマでの結果として評価して、OSと他のがん種の試験の結果を今後は見ていきたいですね。
ツチノコ的には数十年もすればゲノム含めがん治療は完全に個別化される可能性があると思っているので
標準治療の教育体系から変えていかなくてはならないのでは？
という印象です。
