We implemented a guideline for us to do this as well.
Amazing work by former @UCIrvineSurgery @UCI_Trauma Eric Yeates now attending at @HarborUCLASurg @drdevirgilio

In Za we almost never suction and have low rates of recurrence with rapid drainage times. Drains for pneumo out in 48hrs, hemothorax around 3 days even on ventilated patients. CT suction is overrated, and has not passed the RCT.

I’ve been doing this for the last 5 years or so and have a similar favorable experience.

I think we routinely leave CT’s to suction for too long in trauma patients. This reinforces my internal bias but probably need more data for wider adoption.

Equally impressed after I read the review. Things to consider: small sample size, single institution, shorter chest tube time but no decrease in LOS. Still have to read the full paper, but I may give it a try next chance I get.
Great work from @EAST_TRAUMA lit review committee.
This month's lit review has got me thinking about initial use of water seal in patients with traumatic pneumothorax without hemothorax. Any thoughts on this?
The SEAL IT trial challenges the idea that all chest tubes should be placed to suction in traumatic pneumothoraces. 🫁 Researchers found that just one minute of suction followed by an immediate water seal safely reduced total chest tube duration by 16 hours. ⏱️ While hospital length of stay remained unchanged, complication rates did not increase even if a few patients required a brief return to suction. 📉

Medical AI is being deployed faster than anyone is studying it. Johns Hopkins and OpenEvidence plan to close that gap by measuring its impact on clinical decisions and patient outcomes. @FierceHealth has the details.
