NASA-TLX cognitive workload scoring across 103 HoLEP cases reveals that the learning curve extends beyond the first 50 procedures. This study validates the NASA-TLX as a sensitive tool for tracking HoLEP proficiency, suggesting that perceived workload may be a more reliable indicator of learning than traditional operative metrics.

So much hype around @deepseek_ai v4.1 1017. I still struggle with critical workflows, with a hallucination rate double that of the competition. I am sticking with GLM github.com/kindlingai/gr8
Still a work in progress: my first native Zig hybrid built on TensorFold—RTX 5090 + two DGX Sparks.
The 5090 handles attention, KV state and built-in MTP; the Sparks split routed experts over RoCE v2.
Against @MiaAI_lab’s two-Spark lane:
• Single-stream decode: 1.07–1.25× faster in same-window, same-harness tests.
• Fresh-prompt processing: ~1.6× faster in a separate follow-up.
• Four-stream decode: slower, 94.7 vs 100.5 tok/s. First-token latency is worse too.
Hardware/configuration parity wasn’t fully verified. v2.15 remains faster in several historical comparisons.
Out-of-tree, source-only work. More pushes to come.
Thanks to @ashxhart, @MiaAI_lab, @dangerm00se and everyone whose work this builds on. Full methods, limitations and individual credits:

Thank you for highlighting our work! When a surgical technique dramatically improves patients QoL we need to figure out how to make surgeons successful @PECenucleate
An important paper from the US PEC (Prostate Enucleation Collaborative) that compiles experiences and offers recommendations for surgeons planning to learn and perform enucleation. In the USA, hands-on mentorship is less common than in Europe and South America, so learning to perform enucleation is a bit harder.

Patient selection is key, and so is timing. Our new @CUAJournal paper found that HoLEP before prostate RT reduced the bladder volume receiving 70 Gy by about 40% (26.1 → 15.9 cc). Rectal dose didn't change.
doi.org/10.5489/cuaj.9730 @denzel_zhu @urmed_urology @PECenucleate
HoLEP in selected patients with metastatic prostate cancer is not only feasible—it can provide remarkable relief of bladder outlet obstruction.
One week after HoLEP, this patient—previously in urinary retention—is fully continent, hematuria-free, pain-free, driving, and back to his normal daily activities.
The degree of de-obstruction is evident. In experienced hands, HoLEP in carefully selected prostate cancer patients can be performed with the same technical principles—and challenges—as standard HoLEP, while potentially providing meaningful local symptom control.
Cancer should not automatically exclude an obstructed patient from HoLEP. Patient selection is key.

Nice numbers! Different angle on the same 2 Sparks: add a 5090 and split by layer type (5090 = attention/KV, Sparks = experts) → code 83.7, prose 73.1 tok/s.
Engine: @dangerm00se's glm53f-afd (on ds41rt/glmrt by @wrldsuksgo2mars, with TensorFold kernels). Weights: zai-org, Local Inference Lab EXL3, incoai DFlash2. Built with Hermes + Claude Opus 5.5 / GLM-5.3-Flash.
github.com/squarrier/GLM-5.3…
GLM-5.3-Flash on 2x DGX Spark, same weights.
TensorFold vs vLLM:
chat 44.6 vs 22.8 tok/s
code 77.6 vs 41.9 tok/s
prompt processing 1,607 vs 1,448 tok/s
Prompt processing was losing to vLLM yesterday (0.8x). Fixed it overnight.
Found a few more spots with speed left on the table, so reworking that part now.
Anyone else running GLM Flash on Sparks? Have you tried my repo yet?
