Will HK be considering adding Novavax as a choice of COVID-19 vaccine, seeing how it’s on par with mRNA in efficacy, has potentially greater durability, and may also allay vaccine hesitancy and boost uptake by virtue of it being a non-mRNA vaccine? @HKU_SPH @bencowling88
Replying to @Daniel_E_Park
Re: Novavax, numbers are small (~4% of doses), but NVX vaccine effectiveness (76%) may be higher than mRNA (40-45%). Note: values unadjusted, so they are subject to confounding and slightly different from VE in the report.

Six years ago, Beth Dobbin was competing at the highest level of international sport, representing Team GB at the Tokyo Olympics. Now, she is bedbound for up to 22 hours a day after being diagnosed with ME.
Commonly known as chronic fatigue syndrome, the condition has been described by Dobbin as “life-decimating”.
She has written a letter calling on the Prime Minister to meet with her and commit to more funding for research.
Dobbin’s fellow athletes Keely Hodgkinson, Tom Daley and Dina Asher-Smith have also signed the letter. #ME #illness #Olympics

Incidence of new-onset pulmonary fibrosis 4.5 years post SARS-CoV-2 infection in an academic urban medical center
🚨New 4.5-year study: COVID nearly doubles the risk of new pulmonary fibrosis, even in people who never went to hospital. AGAIN, the lung damage doesn’t end when the virus does.
➡️Large retrospective USA matched cohort study(March 2020–August 2024) examined new-onset pulmonary fibrosis (PF) up to 4.5 years after SARSCoV2 infection,
➡️COVID-positive patients (no prior PF) were propensity-score matched 1:2 to COVID-negative controls and analysed using Fine-Gray models for competing risks,
➡️Both groups showed roughly doubled risk of new-onset PF versus controls:
- Hospitalized patients sHR 2.05 (95% CI 1.69–2.49),
- Non-hospitalized patients sHR 2.33 (95% CI 1.88–2.88),
➡️Cumulative incidence remained elevated out to 54 months,
➡️Among those who developed PF:
- Prior COVID hospitalization strongly raised odds of pneumonia (OR 4.84), respiratory failure (OR 3.76), asthma/COPD exacerbations, and pulmonary hypertension,
- Non-hospitalized COVID patients also showed elevated pneumonia, asthma exacerbation, and pulmonary embolism risk,
➡️Sensitivity analyses (cause-specific Cox, inverse-probability weighting, pre-pandemic controls) supported the main findings,
➡️No clear sociodemographic disparities were observed,
➡️The discussion notes that vaccination could theoretically reduce infection rates or severity and thereby lower fibrotic risk, but this remains an untested hypothesis within the paper itself,
➡️Well noted and specified limitations.
‼️So, even mild, non-hospitalized SARSCoV2 infection roughly doubles the long-term risk of clinically coded new-onset pulmonary fibrosis and its cascading respiratory complications for up to 4.5 years.
‼️This is one of the longest large-scale EHR studies linking SARSCoV2 to new-onset PF in both hospitalized and non-hospitalized patients. It elevates post-COVID fibrotic lung disease from a theoretical concern to a measurable, multi-year population risk that can drive repeated pneumonia, respiratory failure, exacerbations, and vascular complications. The findings argue for sustained respiratory surveillance, earlier risk stratification, and proactive management, particularly in diverse urban populations with high baseline respiratory burden, rather than treating infection as a purely acute event. #ProactiveScreening

The strongest immune trace of long COVID in the blood isn't a flare up of inflammation. It's a brake left pressed down. A new study finds the clearest immune signal in LC is raised IL-1RA, the body's own off-switch for interleukin-1 (one of inflammation's main alarm molecules).🧵
Long COVID shows IL-1 pathway dysregulation without a discrete circulating biomarker signature. Most soluble biomarker differences primarily distinguished previously infected from uninfected controls rather than PWLC from recovered. frontiersin.org/journals/imm…

Cyberattack on France’s national blood service (EFS) via its refrigeration contractor Coolworld. Hackers reset freezers keeping blood donor samples at -30°C to a target of 110°C. The thing that stopped it from destructive impact was an on-call technician who noticed the temperatures going up. Frozen plasma intended for transfusion was stored there too. Fortunately nothing was destroyed. This was a cyberattack on a sensitive layer of healthcare. From frozen peas to boiling plasma in three weeks...?

I’ve met a staggering number of adult professionals with long COVID who’ve had to uproot and move in with family just to make ends meet. Others don’t even have that option.
This disease is massively disabling and decimating our workforce. We need solutions, not denial.

A few reasons I think the Washington state #H5N1 bird flu case is important.
1) I believe it would the first human death from B3.13 genotype (once this is confirmed) which was found in viruses transmitting from dairy cattle directly to humans. Will be important to look at whether there are any relevant genomic changes to pathogenicity or if this was more due to the host having underlying conditions.
2) Detection occurred after cows were already sick. We then saw multiple herds get affected quickly and now human spillover. They were not detected through the National Milk Testing Strategy (this is a state-level surveillance strategy w/ decreasing frequency of sampling once you are negative for long enough). This is different than the Daily Herd Status Program which is optional weekly bulk-sample testing at farms which I don't think was happening here. The trade-off of reducing surveillance when you don't have any detections makes sense from an operational/$ perspective but shows that you will then probably miss a new introduction.
3) It is not clear if the infected worker was utilizing PPE at the time of infection. In our research in Central California, PPE use (specifically eye protection) was minimal/none on surveys we did this year. @StanfordCIGH @Stanford_ID

• A trial of 160 people found low-dose naltrexone didn't improve post-COVID fatigue more than placebo.
• Trial will test Paxlovid, valacyclovir and celecoxib for Long COVID.

#LongCOVID recovery depends heavily on the symptom pattern.
➡️ A US NIH-funded RECOVER study followed 1,019 adults with probable Long COVID one year after infection.
➡️ Around 50% experienced sustained symptom improvement over the next three years.
➡️ About 25% achieved complete remission. Recovery was much lower in multisystem LongCOVID: fewer than 19% improved, versus approximately 60% with predominantly smell/taste disturbances.
👉 Key message: Multisystem Long COVID appears to have a substantially more persistent course.
