#POCUS answer: it’s hepatic vein, not IVC. It can fool us in oblique views, when you are rotating from the subxiphoid 4-chamber view to find IVC.
If you see a small, bright walled vessel with a lot of liver on both sides and able to see more of heart than usual, suspect you may not be seeing IVC.
Wall is often bright when the US beam intersects at 90 degrees, that doesn’t make it portal vein. Anatomy is the key.

Just found out that a former resident and practicing psychiatrist took their own life. That makes 5 former trainees I know who have died by suicide.

Hospitals are full of negativity but that doesn’t mean you have to be the pessimistic resident who’s a life suck.

An interesting study was recently published in @yourICM and the authors made some sensible - IMHO - suggestions about how to use echocardiography to guide fluid management in critically ill patients
They recognized 3 scenarios where echo can predict fluid responsiveness (FR):

You and your senior partners (financial beneficiaries of PE transaction) will lose motivation forcing younger partners/associates to pick up the slack
Your younger partners/associates will resent you, but won’t be able to say anything due to power dynamics and fear of retribution from PE overlords
You’ll lose your recruiting edge
You will not practice medicine the way you want, you will practice medicine the way private equity wants
Patients will say things like “why is everybody so grumpy here?” and “what happened to this place?”
Instead, keep your practice, you’re doing great!
