Extreme daily flytipping at the corner of Bromyard Ave & Uxbridge Rd @EalingCouncil . In more or less this state for many months. Needs CCTV + real enforcement. Can you help @Hitesh_T @SteveDW3 @RabiaNasimi @CllrAndrewSteed ?

@TRAID your charity bins here are used by flytippers to justify treating this spot as a local tip. Can you work with @EalingCouncil to come up with a solution?

Wes Streeting as PM will be a mistake. He will be no better able than Starmer to handle the media. He lacks charisma and has been unimpressive as SoS for Health.

The ministers urging Starmer to go are feeding the hysteria. The next leader will be subject to instant leadership speculation and the next, and the next, whether Labour, Reform or Tory. Britain is becoming ungovernable. And the media fixation on this tittle tattle is not helping

If you half the number of hospital beds it doesn’t require a team of management consultants to work out why patients are stuck in corridors 🤷‍♂️
Corridor care is unacceptable and undignified. To end it, we’re sending specialist teams to the hardest‑hit NHS trusts and rolling out new urgent care services. This includes new and improved urgent treatment centres and same day emergency centres. Read more: gov.uk/government/news/nhs-e…

CCT is the mark of a consultant doctor. It is assurance to a patient that they can trust the person treating them. GMC is a flawed organisation with no medical expertise. They want to bestow CCTs to anyone they please. This is a public safety issue. Please speak up against this
The new GMC Order is undergoing consultation. The GMC is seeking to remove the speciality register and decide by themselves who should be issued a CCT. This right is currently protected in law. This is the last hurdle before the GMC will be able to issue CCTs to PAs.

People in the UK need to wake up. This thread explains how the government is using the GMC to remove the requirement for consultant surgeons, anaesthetists, and physicians to be medically qualified. @ShaunLintern surely this should worry you and your readers?
The draft GMC Order 2026 consultation just dropped. Buried in it is a fundamental change to who can be awarded a Certificate of Completion of Training (CCT). This matters enormously. Let me explain why.

This opens the door (even wider) to complete deregulation of medicine in the UK. It will make it near-impossible to distinguish doctors with specialist training from non-doctors who have far less, unaccredited training. This is exactly what @gmcuk were established to prevent.
Replying to @drmattuk
5/ The consultation document even describes CCTs as confirming that “a person such as a medical practitioner” has completed approved training. That “such as” is doing extraordinary work. It explicitly acknowledges the category is no longer limited to doctors.

Most exercise advice focuses on how much you train. This paper shows the real question is what kind of cellular architecture you’re building. This systematic review and meta-regression synthesizes data from 425 human studies to quantify how different exercise modalities reshape mitochondrial content and skeletal-muscle capillarization, two core determinants of metabolic health and endurance capacity. The mechanistic takeaways: • Training intensity is the dominant driver of mitochondrial expansion. High-intensity and sprint-interval training produced ~2–4× greater increases in mitochondrial markers compared with traditional endurance training when normalized for time. • Volume still matters, but differently. Mitochondrial adaptations scale with training intensity × volume, whereas capillary growth depends more on intervention duration (≥8 weeks) than intensity alone. • Capillarization and hypertrophy are not the same adaptation. Capillary density and capillaries per fiber increased even when cross-sectional area did not, reinforcing that vascular remodeling is a distinct biological response. • Trainability is context-dependent. Untrained individuals showed larger relative gains, but well-trained individuals still adapted, especially under higher-intensity stimuli, contradicting the idea of a hard “adaptation ceiling.” • Age, sex, and disease status did not negate adaptation. Young vs. old, male vs. female, and healthy vs. cardiometabolic or pulmonary disease groups all demonstrated meaningful mitochondrial and vascular remodeling with appropriate training exposure. I'm sum, exercise is not merely a behavioral intervention, it is a dose-dependent biological signal that remodels mitochondrial density, oxidative capacity, and skeletal-muscle microvasculature. Intensity determines how much adaptation you get; duration determines how completely the tissue remodels.

One of the most misunderstood parts of medicine: Why health professionals ask you to repeat your story. When we take a history, we’re recognising patterns and testing possibilities. What is causing this? How serious might it be? What can I do to help? 1/n

Wow, I don't think I've ever seen an interview where a government minister has so clearly laid out that student loans are now being used as a way to raise additional money for other public expenditure, rather than just a way to make grads pay for their own education
"So it's alright to change the terms of their loan unilaterally without consultation without pre-warning?" @vicderbyshire challenges Schools Minister Georgia Gould over the government's approach to student loan repayments. #Newsnight

The evidence is uncertain There is logic to its use inasmuch as -aspiration is a significant complication and efforts to prevent it merit careful examination -cricoid occludes the upper GI tract (hypopharynx) -the metrics for correct cricoid are well defined 3 kg one hand unable to sustain > 5 mins -it’s easy to teach (closed 50 ml syringe, compress air from 50-32 ml) achieves good accuracy +/-0.5 kg which keeps in effective and safe range -when applied correctly it has either nil or minimal impact on airway management -if its causing a problem, take it off and you’ve lot nothing The biggest challenge is -teaching it correctly -correctly identifying cricoid ( we should use more USS) -misinformation and a view that absence of evidence equals evidence of absence of benefit Embrace and accept uncertainty So I use RSI in high risk cases
